Reasons for green/blue mold on the cake

Hello, I realize green/blue mold patches on the cake are nothing good. I tried to be as sterile and careful as possible. Even some forum members had a laugh at my OCD approach to sterile environment.
What may be the reasons for the mold on the cake? What are key things i should or shooed not do to prevent such contamination?
Please no comments like "Your cake is fucked" etc. Such comments dont introduce constructive feedback at all.

You are always the reason for success or failure in mushroom growing. Either because of choices or skill.
Don't use spores directly to inoculate. Spores to agar. Agar to grow.

It seems some little heads are showing up. Is there anything i could do about the mould? I wouldnt like to damage pins and some little shrooms by removing the mouls incorrectly. Should I be touching it at all? Perhaps just wait and collect whatever will grow?

Just play it by ear. A lot of grows are some combination of success and failure. You got some success going there.

Update to my first ever cult story. First flush was ok, i suppose. Well, i got some fruits 
Now, I have second flush - this time, after cooling cake in water, in the fridge for 24h caused the mold to go away, leaving beautiful cake.
Although, first flush fruits left some spored on top of the cake. So majority of the cake is not fruiting.
My plan for the 3rd flush is to simply cut off the top of the cake, where the spores dropped. Like cut off a loaf of "bread".
I hope it will work. For now, my second fruiting in progress:

Yeah cubes always seem to find a way. I left a shoebox with mold just chillin. Never harvested first flush mush and just let it roll, it’s on its fourth or fifth flush now with nice clean cubes popping up between a forest of mold infested and rotten earlier flushes.
Golden dude just FYI, spores dropping has no effect on anything except for making a bit of a mess. Cutting into that cake wouldn’t do anything good and might shake off the good luck you’ve had so far with it.

Don't cut into it. Starting out with little to go on and getting any fruits at all is a good sign. Keep practicing. There is no "wrong way" but there are definitely ways that work well consistently. Keep practicing and you'll eventually come up with your own.
Take spore prints from your good fruits so you can keep going on your own. A good spore print when used well is enough to last a lifetime but having a good handful to work with is great in the beginning as there are bound to be failures and that's ok.
