It’s already been mentioned but venting your PC is a must. If it’s a type of PC you can’t vent then I just don’t know, I’ve never used one like that.
The mold looks like it’s showing up all over your jars and not just on top (suspect bad filter) and it doesn’t look like it’s coming from the areas with wedges (bad inoculate) so not fully sterilized grains is what it looks like. No venting mixed with 14 psi max and only 90 min could be the culprit, although mold spores die at lower temps than bacterial endospores.
I do 17-18 psi for 90 min but if I could only get to 14 I’d do at least 2 hours, probably 2.5

i always do 2.5 hrs. i used to get away with 90 minutes, but after bumping up the PC time, i started getting waaayyy better results

I can basically set it on 2 different modes, but I can also remove the cap thingy on top which kind of acts like a weight and this would allow me to vent it before putting it on 14 psi. I'll try it out anyways, thank you for letting me know!

I've got one of them, or at least very similar.
The gauge goes from: open 1 (I want to say 7psi?) 2 (I want to say 12psi?)
Leave it on open to vent and (using an oven mitt or similar!) twist it to 2 after 10 mins of constant steam and start your timer. 
Def PC for at least 2 hours with those as they for sure aren't 15psi...

I lost almost 27 of mostly half-pint / pint jars to green mold, will redo them over the next few days and make an update on how it went with this new approach

I would also recommend sterilizing your tool when you do your grain and switching to using a single wedge to inoculate so that you only open each jar once and only your blade and the wedge go over the open jar.

As if putting the scalpel in the PC?
I kind of sliced the petris into a square net and then scooped the wedges into the jars when I did it, thought it will colonise faster No rush with this hobby though I suppose

Yes. I made a tinfoil envelope for mine and run it with whatever I am PC’ing before doing sterile work as an extra precaution.
Extra wedges do mean extra inoc points, but if you are leaving the jar open, hovering over it with the dish while scooping out the wedges, or opening it multiple times in order to get them in, you are increasing the opportunities for competitor species to get into the grains as well.
