Most surefire, idiot proof way to grow Mushrooms?

Hey Guys,
So let me start off by saying I'm by no means a beginner anymore. I've been trying to grow mushrooms for 6 Months now, but never successfully.
I bought a growkit out of frustration a month ago and it worked ok, I got some mushrooms out of it at least.. but I wouldn't call it a success. Also I only bought it to get some different genes and grow from there. So I cloned the best looking ones on agar and made an LC. I then proceeded to inoculate grain spawn and BRF Cakes with it. Also I inoculated two Grain Spawns with the agar itself.
The last 6 months were unsuccessful almost exclusively because of bacteria.. I just don't get how to avoid getting bacteria in my spawn or LC. And the current run isn't looking so well either (the symptoms are slow growth again).
The agar slices inside the spawn aren't doing anything besides colonize the local 2 square centimetres and then stop for some reason, which doesn't make any sense, because slices from another almost identically looking agar plate colonized the LC perfectly.
I'm getting frustrated and have a few questions:
-Why do my agar slices colonize the spawn so slow and almost stop at some point, even though everyone tells me it's the safest method? (The agar slices are 100% clean)
-If BRF Cakes are easier for beginners, how do I get clean LC 100% (which to my knowledge is the only good way to inoculate BRF Cakes) ?
-How can I spot bacteria early? The older grain spawns looked perfectly clean and colonized in 10 days, but were contaminated with so much bacteria, I didn't even get a single pin, which I only found out later.
-Is there any way that my tap water is contaminated with bacteria, even though it is rated as drinkable?
-What is the safest, most surefire way to get clean spawn and/or a clean BRF Cake for sure? I've got all the necessary equipment at hand, except for a flowhood, but a still air box should be safe enough, right?

Agar left on top of a grain jar can dry out before colonizing. Bury it under the grain.
Cakes made with brf water and verm are bacteria resistant and made for spore syringes specifically.
Liquids can become contaminated easily. Lc and Li are advanced techniques. Stick to agar, grain to grain and spore syringes for cakes.

If your cultures stop growing on agar they are apparently not clean or otherwise no good.
Also, if you haven't had any success yet, stop using lc for now and inoc with agar wedge. You do transfer a few times right? (one that keeps colonizing tho or it is just not good)
