No second flush/Small Yield

Newbie grower here. I am at the end of my second attempt using PF tek with BRF cakes. I have succeeded with making my own substrate, inoculation and colonization. I have a SGFC made to spec. On both of my attempts I have ended up with a very small yield (2g dry) from 6 cakes and both times I never got a second flush. I did do a dunk after the first flush both times for 24 hours in the fridge. I fan and mist 3-4 times/day and try to avoid spraying the cakes directly. I know how difficult it is to troubleshoot someone else's issue without a super detailed description of exactly what procedures were followed but I'm hoping someone can give me some clue as to what would cause such small yields and no second flush with BRF cakes? Thanks in advance!
I used the 2:1:1 ratio for vermiculite:BRF:water for the cakes. One time I used 8 oz jars and the other I used 16 oz jars and got the same results (small yield and no second flush). I think I am doing something wrong after the cakes get in the FC (which I hear is supposed to be the easy part). No contamination issues that I am aware of in the jars.

If you've gone from a multi spore srigney then it could just be weak genetics.
afaik the dunk doesn't need to be in the fridge.
Also have you read up on the misted-cake project and surface conditions? It's best not to mist and fan to a schedule but to let the cakes tell you when they need more water.
How long did the cakes take to colonise? And how long did you leave them to consolidate once the were 100%?
Maybe next time try using you cakes as spawn for a shoebox and see if you get better results?
Also pics of your set up would definitely help, more experienced people may chime in, but they will also probably like to see a pick or two.

Thanks for the feedback. Here are some pics of the setup. I taped off half of the holes on the chamber with micropore tape which admittedly may be part of the problem. It was an attempt to maintain humidity in the chamber during a grow attempt during the cold, dry winter. Also, should the plastic sheets underneath the cake be trimmed to be smaller? I will take a look at the misted cake project. I think that will be helpful.
I used liquid spores from a reputable source. They were stored in the refrigerator before I used them (not sure if this is necessary or not).
It did seem to take a long time to colonize the jars. I forgot to mention that these substrate jars were from Midwest Grow Kits and I did not make these. http://www.midwestgrowkits.com/ultimate-pint-substrate-jars-6-pack.aspx
I left them colonize for 8 weeks because it seemed like they never fully colonized. Here are some pics right before I birthed the cakes:


Here are some pics of the setup. These pictures were taken today which is 12 days after the second flush dunk. I'm probably going to toss the cakes today unless there is any hope for these.

I forgot to mention that both times this has happened (small first flush, no second flush) I used spores from two different (reputable) suppliers so I think its unlikely its due to bad genetics unless I just have terrible luck. Also, the first time it happened I used my own cakes and got the same results as I did during this grow.
