What is causing this.. San Pedro

The scar or the thinner growth?
The scar is nothing to worry about and the thinner growth is just from when the cutting was taken and the plant didn't have the benefit of a fully developed root system / old growth when it started growing. Future growth will be thicker as the plant grows and the base will get bigger as well, but the size ratio will remain the same, it won't "fill out". All columnar cactus do this, nothing to worry about. Feed em, love em, watch em grow big.
The thinner growth will also come about near the end of the growing season when sunlight isn't as intense. You might see this pattern on older, well established plants where the growth thins a little bit, then gets thick again. Most of my plants are doing this right now because there isn't as much light but the temperatures are still high, so they still want to grow.

Oh wow thanks so much for the help! I had no clue.. theres also some yellowing, and dots. Is this over watering?

I can't say for certain, but I would suspect it's from a bug that has been sucking on the plant. Lots of stink bugs, leaf-foot bugs, and cactus bugs in the south. The cactus bugs are usually limited to prickly pears.
