its kinda messed up, but this did happen to me before and i did case the last one RIGHT.. this case:

damp verm - bottom (only about 3/4")
mycelium layer - middle
damp manure - top .... then after i seen the patches coming up.. i patched with vermiculite..

hey i just read this in another thread... its by Aeolus1369...

"Casing serves primarily to provide moisture and a higher fruiting surface area."

the "higher fruiting surface" would that mean that the shrooms would be bigger if u cased with more top layer of vermiculite??

oh ok, so i should stick with the theory on a bigger bottom layer (substrate) will make them bigger fruits?

Bottom layer is ok.
Middle layer and top layer become as one because both are a substrate with nutrients, therefore all you did was spawn your cakes(assuming) to the manure. You should have let this colonize 100% before attempting to fruit or apply a casing layer.
You also patched with a non nutrient ingredient,so really you had a 1/2 assed cased casing. Not to be a smart ass-but you only cased what you patched, the rest was manure,You should have applied the verm to the whole surface as a casing layer and patched the growth as it came through that layer , with the same verm.
But manure will fruit without a casing also, from what I have read.I always case.

let me put this in noob terms so i understand better lol...

bottom layer 1" damp verm,
middle layer - mycelium
ontop of mycelium layer - damp manure
(let colonize manure 100%)
then add another top layer of damp verm, and when the mycelium starts poking through the top verm layer, then patch if needed, and fruit WHEN? just wondering.. when u see the mycelium coming up ALLLLLL OVER right?... (without having overlay)

What were your temps in fruiting? It's heading to summer in the northern hemisphere and I'd hazard a guess that many places up there are already getting warm. Temps last summer downunder were very high, 28C to 30C fruiting temps happened my way -- as per seen in 1st pic. This caused smaller than desired fruits, i.e. the stems were generally less meaty on trays.

ok doc, so when it spawns 100% colonizes.. then i case, i know this.. but wut im askin, is when do i move it to fruiting temps?... when it starts to poke through, or right when its 100% done, after verm is added on top?

When you start to see the mycelium coming through the casing, everywhere, meaning in all the valleys-It should resemble this!!!

Then you fruit it!!!

They'll fruit off uncased colonised manure (they did outdoors for me in manure flower pots).

But if you case (I would) do so once the bulk substate has fully colonised. To keep the Co2 high, just put some cling film or foil over the top of the manure (in the pic, it's straw and manure).

Once it has colonised, case it. Put the the casing layer on level (anyway you please). I didn't bother patching this one so the pinset/fruiting wasn't optimum, but once it has mended through the casing layer, introduce to light. You will anyway, by just inspecting it as it will get enough light to induce pinning. Some plastic sheet over the top at room temp is fine.
