Troubleshooting PE tub...help!

I've been reading the forum for a few months now, and have been thankful for what I've learned here. Didn't think I'd run into a problem where I'd need help, but here I am! I did do many searches to try to find the answer, but it's possible I don't know what to search for. So far, I've completed 24 pf tek jars, with decent results and 0 contams. Yay! (This includes a few jars of PE, that actually yielded better than some of the others, averaging around 8g, dried.)
Here's where I'm at now- I'm working on a PE monotub, and some weird things are going on with the fruit. It seemed to colonize fine. Spawned rye that had been inocluated with a PE LC (that had been used on cakes with no trouble) that fully colonized in 11 days. Substrate used was a coir/verm mix at field capacity, with worm castings added, and pasteurized for 90min in the oven. Cased with more coco coir once fully colonized and set to fruit. Pins popped up in apx 12 days, and looked decent. Significantly smaller than what I've been seeing on the cakes, but not super terrible.
However...shortly after veils tore, they...stopped growing. They never completely opened. I gave them a little over a day, but no progress was made. The stipe began to look grey-ish, so I harvested everything there for fear it would get worse, and the stipe was even less dense than I've become used to- almost mushy feeling. I cut a couple open, to see what was going on, and they were dark on the inside, as if they just stopped growing and started rotting while still connected to the mycelium. So weird... what could be going on, here? The mycelium otherwise appears healthy. Are these safe? Should I trash it? Help, please!

For reference, this is what the tub looked like just days prior- they all looked healthy, although small. (Light source was to the right...)
Also, adding in a pic of one of the PE cakes, which produced MUCH better, which is more along the lines of what I'm used to, in my limited experience. This was the first flush, which produced 52g, fresh.

Blotch baby. Need fresh air. Also fuzzy feet. That was your first sign. You didn't see it, so blotch hopped in to teach you. Nice job though. Keep it up.

So... do I need to scrap this, or can I try for a second flush? Is what I have OK/safe?

You're good. They for a 2nd. They are good. Just dehydrate completely.
