The mold I would say is from bad lids or improper sterilization or unclean agar. Don't use anything contaminated to inoculate, take transfers until it's clean.
The slimy jar is bacteria. Can you tell us about your process?

The moisture of the grains could be your problem, I'd throw any that have the green Mold. Only use clean cultures, if theres bacteria/Mold transfer to a new dish until clean. If this bacteria is appearing long after the mycelium has started to grow, what's your agar recipe and how do you store the plates?

Sorry bud I would start over. That grain jar is no good. Gonna get a bunch of mold if you grain2grain it. Your process sounds fine just need some more practice. How long do you pc the agar for? Also you only need to pc oats for 2 hours

You might be able to take a few clean looking grains from the top but you might waste more time. I would take a clean looking colonized oat and put it on agar and transfer until clean. Maybe up the pc time to 20-25. What temps you keep the agar and jars at?

Increase your PC time for your agar to at least 20 mins.
Pour slow and steady. Inoculate slow and steady. whats your SAB build look like? How are you transfering? Whats your sterile tek procedure? What PC are you using? How long was the cook of the grain?

You can take a clean grain and transfer it, but transfer the resulting myc on agar VERY soon after its about a cm away from the grain. Do at least 2 transfers on everything from here on out.
My money is on your grain not being prepped and cooked properly. Follow a tek for your kind of grain to the letter. Make sure youre venting your PC for 15 minutes with steam coming out BEFORE you cap it with the rocker, and only once its up to pressure and rocking a lil do you start the 90minute timer. maybe go to 120 minutes as well.
Also, dont bump a thread after like 15 minutes of posting, this isnt discord.

I wouldn’t count on having much success going to grain until you’ve got your contamination rate way down on plates. Just reread threads on sterile technique and SAB stuff.
As you found out it ends up wasting a ton of time cuz the jars usually don’t go bad until at least a week in.
I hate losing jars especially after weeks.

I would take the yeast out of your agar recipe, pc a little longer, Make sure grains are dry before PC, and use a metal scalpel dude lol. I don’t know how you could get an alcohol dipped plastic scalpel red hot...just get an exacto knife if you really don’t wanna order a #7 scalpel (the bees knees) and make sure you wrap your plates in parafilm or modified Saran Wrap.
You should be able to transfer just 1 or 3 times and have a totally clean agar plate. And like above, I wouldn’t try to g2g that jar. There’s probably more mold in there that hasn’t sporulated yet so it’s still hiding in white.

I’ve never seen it in a recipe but I’m pretty basic with agar. LME or grain soak water for the nutrition.
And for the grain prep I haven’t read bod’s oat tek but the more hydrated the grain is the more dry the outside needs to be. So if you soak a long time and then boil for a bit or whatever they should be dry on the outside. No need to go crazy just not having them stick to your hand. For WBS I’ve been just dumping the grain in boiling water, take it off the burner and put lid on, let it sit 20 minutes, then strain for 2-3 minutes to get the big water out and loading in jars. When they’re more dry on the inside you can leave some water on the outside so it soaks in during the PC.
