ISO Serotonin Syndrome info
    #26762906 - 06/21/20 11:58 AM (3 years, 7 months ago)

Does anyone have any links, essays/articles, or book recommendations on the subject? Specifically SS induced by taking psych meds on a regular basis and mixing with psychedelics.

I had an extremely difficult experience a few years ago that I'm still trying to make sense of. After researching on and off over the years, SS seems to fit the bill. I'm entirely open to any other interpretations or opinions of the experience.

Symptoms experienced:
Muscle tension
Muscle spasms/twitches
Mini black-outs
Black-out (coma quickie?)
Seizure
Confusion
Delusions

Substance:
Supposedly LSD. Didn't have a taste, but it did kind of fizz when we dropped the liquid onto a marshmallow (?!)
Didn't have a test kit at the time.

Meds:
Seroquel nightly
Lithium daily
For at least 4 years up to that point. I have since tapered off (with guidance from my psych) and have been medication-free for around a year.

I ended up in the hospital with no memory of paramedics arriving, the ambulance ride, or intake. I just "came to" and was confused as to what happened and how I got there. They must have flushed my system because I sobered up steadily over the course of a few hours.

The rule of thumb is simply do not take psychedelics if you use Lithium.

the maximum stoned effect is always blackout. nothing strange in that, in having no memory of how you got there etc.

after you do not take these medicines you can reconsider psychedelics. The two initiatives, psychopharmacology and psychedelia are at cross purposes.

I don't think it's likely that what you experienced was because of serotonin toxicity. What did the doctors/nurses say the issue was while you were in the hospital? What was the reason you were taken away in an ambulance? Who called the ambulance for you?

From my basic understanding of things I just don't see how serotonin toxicity could occur from any of the classic psychedelics. Dig around online and I bet you won't be able to find anything "official" about people experiencing serotonin toxicity from any of the classic psychedelics. Just a lot of anecdotal stories of people thinking that's what happened to them.

I've tried to find documented cases of it happening and I can't find anything.

Redgreenvines: I am no longer on those medicines. Hadn't heard that rule of thumb before, thanks for the info. I haven't had any psychedelics since tapering off, either. Anything I may become interested in will still be approached with caution.

Openmind: That's almost a bummer to hear, as I yearn to understand what happened that night. Nothing like that has ever happened for me before. It was beyond out of the ordinary.

My boyfriend at the time called an ambulance for me because I was non-responsive and convulsing at irregular intervals. Our neighbor was a nurse, so he got her first. After she saw me, she said "there's nothing I can do for her, call 911."

I've done a bit of research online and haven't turned up anything incredibly helpful. The closest I've got were descriptions of SS and it seemed to match up with what I experienced. I may very well be grasping at straws here. As I said, I'm quite interested in cracking the mystery of what happened that night. In reality, I may never know.

The Wikipedia article for serotonin syndrome does list both lithium as a drug that can contribute to SS, so I think you might be on to something. LSD is a known risk factor.

SS seems to be a condition that not many doctors are even aware of and I'd imagine it's not always diagnosed correctly. I once had to explain to a doctor that I couldn't combine two medications due to the risk of SS. He then pulled out his phone, went to WebMD, and basically said "Huh, you're right". Yikes!

If you took both lithium and LSD in large enough amounts, then SS definitely seems to be within the realm of possibility. The muscle spasms and seizure really point towards that IMO.

If you're looking for more information, I highly suggest Google Scholar. There's a surprising amount of research papers about psychedelic drugs available there. The reading can be a bit dry since it's medical jargon, but the information is invaluable.

Disclaimer: I am not a doctor, take everything I say with a grain of salt. Batteries not included. Some assembly required.

TeaAndCrumpets: Thank you for your input. I was on a regular daily dose of Lithium for around 4 years up to the point of that difficult trip, so there was likely a substantial amount of it surging through my system (I got my blood drawn to measure the Lithium levels every few months). The dose of LSD was entirely unknown, as it came from a liquid dropper bottle. I don't remember the size of the dropper, which would have a hand in the actual dosage of each drop.

Thank you for the recommendation to Google Scholar. I will have a look around there to see what I can find. I also agree that not every doctor will have the necessary knowledge to accurately treat and/or diagnose certain patients.

Openmind: I neglected to answer your first question about what the nurses/doctors had to say about my condition at the time. The answer is not much at all. I was not forthcoming with what I took to put me in such a state, I only told them about the medicines that were prescribed to me.

During the difficult trip before I was taken away by ambulance, I went searching for my anxiety medicine (which is prescribed as needed, hardly ever took it). When I couldn't find it, I went into my Seroquel stash. I had 100mg pills prescribed to me at the time, which I would chop into halves and fourths to take at night. The pieces were kept inside of the pill chopper and I spilled them all over the kitchen counter and floor when trying to get some out. The EMTs made note of the chopped pills all over the place when they picked me up.

I explained to someone at the hospital that I had a massive panic attack and took more Seroquel to help me get to sleep as I couldn't find my anxiety medicine. I didn't admit to drug use as I was in a bad way at the time. So they didn't have much to go on and I wasn't giving them any more information.
