I'm talking about harris county

most of texas is as bumfuck as anywhere but we (houston) technically do have the worlds largest medical center and research complex in, well, the entire world. it would be a bit surprising if the medics here didnt manage to keep the CFR lower than in other places.

edit: nvm CFR here has jumped to 2. but I am telling you... we havent been testing almost anyone. i personally know someone who was very sick with COVID19 for 2 weeks and it took her 9 days to get approved for a  test  because she is younger and by the time the results came through she was already better again. when all this shit first started happening I got a notice around march 10 that I had had a patient who later tested positive for covid19 but we didnt know it in the ER, had no PPE on whatsoever.. they asked me to quarantine but then 4 days later reneged, asked me if I had any symptoms, I said no, they told me to come back. was never able to get tested.

the number of cases is far far higher than being reported, but the number of deaths isn't. probably a few people dying at home here and there but not many from COVID19. which means the actual CFR is below 2.

Apparently Harris county has some severe issues with not having an accurate count though. It’s all over your local media.

I would not believe any CFR that low in the US. It’s simply not consistent, especially with the level of poverty in Houston.

Houston has the highest rate of uninsured people in the country. These are people who aren’t going to a hospital even when they are very sick because they can’t afford to. They end up dying at home without ever getting diagnosed. They never get counted.

the media in general seems to be operating on a weird filter/lag-time. the medical treatments they discuss, the interviews they conduct- they all seem like they came straight out of 3 weeks ago in the actual world.

I guess a lot of them don't know. Many of them do, though. I treat uninsured patients every single [work]day. At least half of my patients are uninsured undocumented immigrants with limited English (which I love actually, they tend to be very grateful with the quality of services they receive). In the ER if you present with a life threatening problem you receive treatment for it. It's like, the law? 99% sure. Hell, if I'm in public and someone starts dying I am legally obligated to try to help. I'm not pulling out their insurance card before I start compressions :lol: I've never understood all of the financial stuff but the hospital figures out ways that these peoples' acute care treatment gets paid for. If they can't pay, they don't pay. Now, we end up prescribing insulin and all kinds of stuff that they definitely cannot afford when they leave, but that's a different story.


Of course, if you want to get treated BEFORE you're at immediate risk of dying, you'll need insurance.

It’s not limited to Houston, but Houston has particularly bad healthcare access for an unusually large poor and unhealthy population and I am 99% a 1.4% CFR is total bullshit.

I’d be curious to see data on deaths of unspecified or natural causes in the past month. NYC for example is seeing 8x the rate of people being found dead at home or on the street. 1100 in the first five days of April and I doubt they will get counted either.

It seems that the course of this disease is that people are sick for a week, and if things go bad, they go bad very quickly. I’ve seen many reports of people coming to the ER and the doctors being shocked how quickly they need to go to the ICU. If you are someone who is putting off going to the hospital until the last minute, that is a recipe for disaster because outside of a critical care situation that kind of disease progression is rapidly fatal.

Definitely. this is a good time to own a pulse oximeter

and yeah i will take you up on that bet. I'm betting by the time we reach zero cases the NATIONAL CFR (including positive antibody titers and not just tests in the total case #), will be 1.8% or less and the CFR in Houston will be 1.4% or less. I wouldn't even be surprised if it's as low as 1%. That is still 10x as deadly as the flu

I think this must just be a matter of ignorance :shrug: there are a lot of services. the city gives free immunizations, provides very cheap public dentists, has cheap RX plans, we have two public teaching hospitals where 80%+ of the patient population is uninsured :shrug: you definitely get access to better doctors with insurance but a lot of people just... avoid going to the doctor.

The CFR in Houston is .6. The city with the highest rate of uninsured of any major city in the US has one of the lowest CFRs in the world. That is so laughably not credible.

I’ve been saying for weeks the true CFR for covid is at least 1.3. I used to think that was a good estimate, but in realty it is probably closer to 2%.

One thing about these statistics that is consistent everywhere is that the CFR starts to get much higher after the peak of new cases. The deaths continue to mount as the rate of new cases fall. People were shocked when Italy was at 6%. Now they are pushing 13%.

This is honestly a really really tough call for me to make. I think this disease is causing people to drop down to levels we would normally intubate for even though those people go on to recover without intubation and apparently sometimes dont even appear to be short of breath. If you know you have heart disease, liver disease (hepatitis/cirrhosis), anemia/sickle-cell disease, moderate to severe asthma or COPD... I wouldn't even wait for my sat to start dropping. The onset of fever is typically before the shortness of breath as I understand it. I would go when I have the fever.

If you've got a healthy BMI, don't have hypertension or diabetes or chronic kidney failure or hepatitis etc... eh, 94 is when I would personally start to get worried. The rate at which it is dropping is as or more important as the absolute number. If you've been sitting at 95 for 3 days and feel okay, you probably will be. If you were at 100 at 6pm today and you're at 95 at 7pm, take your ass to the hospital immediately.

I hear what you're saying koods. I stand by my predictions for the moment. I personally don't find it surprising that the CFR here is either lower or at least not higher than elsewhere simply because we have a lot of uninsured people, given what I believe to be real differences in quality of medical care rendered to patients in Houston (go poke around the houston medical center, the affluent from all over the world- especially saudi royalty- seem to agree that it's worth flying to Houston and paying cash when you really just wanna avoid dying at all costs and all the doctors at home say it can't be done).


you can boo hoo remdesivir all you want. almost certainly nothing will ever prove to cure this disease, it's a virus. the whole conversation about how there are "no proven" treatments for this has always irritated me. a drug that increases extubation rates from 20% to 57% is worth taking a look at. yes it was one small shitty study but I feel like you are sort of intentionally gutting nuanced analysis just to be a doomsday prophet.
