Issue with jars

Having had a tub be devoured by mould I have had ongoing issues with possible bacterial contamination.  I just thought I would see what other opinions people have?
I get these condensation droplets and sometimes grains that don’t want to colonise?
Is this due to grains being too hydrated?  I have even seen this with master jars with a clean agar wedge and pressure cooking for two hours.
Any thoughts?

I'm thinking possible moisture content issue. Maybe hidden bacteria on agar.

Make trays and put them in a shotgun fruiting chamber so you don't risk losing everything at once.

Also, are you using synthetic filter discs so the mycelium gets enough oxygen to successfully colonise everything?

How are your temps?

Definitely never had any issues with monotubs in the past -
We did have some hot weather, but that has past and there was some heat stroke after moving house - but having gone back to agar several times and not seeing any issues on the plate I figured the process must be okay
Current temps are around 25C or 77F.

Using SFDs as well, following the process outlined by RR to prepare jars.
My agar cultures were a couple of months old and stored in the fridge.
Totally confused how this could come about?
