Bulk substrate won't properly colonize. Have tried different "recipes"...

Hey all!
I've grown multiple PF-tek harvests with good results but still consider myself a beginner when it comes to growing. PF-tek sure was easy to handle but i wanted to try Bulk growing mainly because PF-tek is such a hassle with a large amount of cakes... I'll try my best to explain my steps when growing and the results that followed:
- Original Cambodian Cubensis spores are from a well known online shop. Since then i have used homemade spore syringes, G2G and LC to inoculate my oats. (Oats sold to feed horses)
- My tubs are 50qt and each have 6 x 1.5inch ventilation holes. I have covered the holes with varying amount of micropore tape to see if FAE makes a difference. Humidity has varied between 80-100% in different stages of trial and error... Ambient temp is around 70F. Liners i've made out of black plastic bags. I've tried with a light turned OFF first and then at a 8/24 cycle. (6500k).
- My bulk substrate i have "ghetto pasteurized". Meaning pouring boiling water on the ingredients until the coco is moist and everything is mixed. Then leave it to cool for several hours with the lid on until it doesn't feel hot to touch. Oat to substrate ratio has been between 1:3 and 1:4. Didn't seem to make much of a difference... Depth of substrate is around 1.6 inches. As casing i've used just enough of the same substrate to cover all oats.
The different recipes of the substrate are in the picture below. 4 ingredients used: Coco coir "Ko" (Measured in dry brick form) Vermiculite "V" Used coffee grounds "Ka" (Also tried without any coffee) Gypsum "Ki"
- All these tubs have had plenty of time to colonize (~1 month). Only a couple tubs have resulted in a few stunted mushrooms. The "substrate casing" has hardly colonized at all. After digging up the failed tubs i've noticed only ~1/3 of the substrate has colonized under the casing.
- A couple tubs i've lost to contaminations (trich) but that doesn't seem to be my main problem.
I haven't followed just one TEK. I've read many sources/TEKs and compiled things that i can get my hands on or have had experience with growing PF-tek. I'm at a dead end now... Can you point me to a reliable TEK that uses the "ingredients" mentioned above or BEST situation point out a mistake with this method that i've already used.

Personally if I am going to supplement a substrate I would pasteurize properly. If its straight coco coir I can get away woth the ghetto past.

Coffee has no place in substrate IMO. And if you use coffee in your substrate you have to pasteurize it the perfect way not the ghetto way.
Also it sounds like your spawn is bad

Sounds like the pasteurization is one thing i need to change. Will do it in a big pot from now on. My 14qt pressure cooker pot should be big enough.
So even though the spawn seems to colonize normally in oat it can still be "bad"? As in not healthy?
In my first bulk grow i used ground PF-tek cakes that i didn't want to use the traditional way anymore. Out of that i got 1 decent flush. Not great by any means... With oat spawn i haven't been able to get even that amount of shrooms.

Yes especially if you inoculated directly with spores. If you made an LC from spores expect ridiculous amount of failure If you inoculate grain with spores expect medium levels of failure.
