A lot of guys in the gym who are 6'0 225lbs will say stuff like "BMI doesn't pertain to me. Yes I have a BMI of 31, but since I'm 13% body fat, that's not a problem."
I had always kind of assumed that they were probably right to say that. It seems they may be mistaken.
Does Lean Body Mass Equal Health Despite Body Mass Index?
'The purpose of the current study was to compare individuals with high BMI, but with differing body composition, to determine if there were any differences in the likelihood of having cardio-metabolic risk factors, MetS, and diabetes. The results of the current study suggest that regardless of LBM levels, the likelihood of having cardio-metabolic factors is identical in all individuals that have a high BMI ( 30 kg/m2), thus reinforcing the strength of BMI as a clinical tool for the management of obesity and its related cardio-metabolic risk factors.'
The wording could be better but essentially it says that lean body mass is not a significant predictor of reduced adverse health events in individuals with a BMI over 30. Basically, weighing too much may be a problem even if the weight mostly consists of muscle.
If this conclusion is borne out by further research, it will come as a slap in the face to many who spend time doing resistance training for muscle gains. This is only one relatively small epidemiological study, however. I'll be interested to see if the results can be replicated with a larger data set. In the meantime, I'm feeling a little less ungrateful I seem to be stuck at a BMI around 23 no matter how much I work out or eat.

Anything that adds blood vessels to you body makes it harder for your heart (IMO). Whether it's fat or muscles.
If my theory is correct, in the point of view of the heart of TheRock, he is morbidly obese.

It never seemed healthy to get extremely ripped. iirc, full time body builders have low life expectancy

That, I suspect, is a spurious correlation. Body builders tend to use substances that increase blood viscosity, blood pressure etc which can lead to adverse cardiac events.

It's crazy to me that BMI is still in use today. The formula seems fairly arbitrary.
