Rye grain/CV contaminated 10 days after spawn

Hi guys, one part of my shroom project definitely failed. Maybe you could help me identify my mistake.
I took rye grain, soaked it for 24h, simmered for 15 Min, let it dry, PCed it in jars with a 1/4" hole and a polyfill filter. I then had 4 quart jars 3/4 filled with good looking rye.
On 11/2 I inoculated the rye jars with an ecuador MS syringe, 2,5 ml each.
On 11/17 at 100% colonization, everything looked fine, no colorations or anything contam-like. I spawned to CV with a ratio of 1:2 spawn:sub. I used unbucket tek to prep the coir and baked verm in the oven for 30 minutes at about 350 F, and mixed it at field capacity in a really clean and desinfectant-wiped down environment, with clean bare hands and put it in 4 shoeboxes.
On 11/27, 10 days later, the surface was bright white all over, maybe looked a bit too fluffy... at that point I noticed a sour smell in one of the boxes and put them all to quarantine, as I have pf tek cakes as well.
2 days later, the surface had some green and grey spots. One by one it all went to shit.
What do you think was the factor that destroyed my very healthy looking spawn? Did i rather fuck up the colonization already or rather when spawning to bulk? I mean, I didnt notice anything wrong before the smell came up. I just don't want to make the same mistake again.
I have some ideas: A. Shaking the jars (with a polyfill filter) midway of colonization contaminated the rye due to thr rye touching the polyfill on the inside B. The coir still had contams after mixing it with (very) hot tap water C. My hands were not clean enough mixing sub and spawn D. I fruited the mixed shoeboxes too early, at about 75-85% colonization and fucked up something there
Whats your guess?

These are from the day of spawning and 4 days later:

The foil on the small box had some small holes for GE, the other box had a proper lid and no holes. When i fruited them, i put the small one in a shoebox on perlite with the lid upside down. With the big one i just turned the lid.

Grain looks ok I’ve seen a lot worst on the first pic is there sedan on top looks like something is over the container, if so I would look at put those in containers in a mono. The containers don’t look contaminated to me looks like normal myc
