http:// Habitat: Where does it grow? Eg. woods, pasture, state, province, country, altitude, etc. What does it grow on? Eg. soil, dung, wood (dead, living, what kind of wood?), etc.
Gills: Color, attached/not, gills/pores, etc.
Stem: Length, diameter, color, texture, hollow/solid, thin/thick, etc.
Cap: Diameter, color, texture, conical/spherical, convex/concave, etc.
Spore print color: Very important!
Bruising: Color that the mushroom bruises, if any.
Other information: Scent of the mushroom, anything else you think is important, large close-up pictures showing stem, cap and gills.

It would help us to help you if you would actually fill in the template that you posted. In order to identify certain mushrooms we need to know some basic information along with clear up close pictures of the cap, gills and stipe (stem) taken preferably in natural light, in situ photos are very helpful. A general location such as the nearest city or town, if in situ photos are not provided we need to know what your mushrooms were growing from (wood "living or dead", wood chips/shavings, grass, soil, dung etc. Color of the spore print and any bruising where handled or injured and with some species it's important to carefully dig up the entire stipe base and look for certain characteristics such as a cleft bulbous base or saccate volva, characteristics such as the ones I listed are all important parts of the identification process.
With all that said, I cannot accurately identify your mushrooms to either Genus or species, but someone else may be able to help you.

They were growing in grass in my front yard, and I’m pretty sure these aren’t anytime of Magic mushroom im just wondering is these are poisonous or not because my dog ended up eating some of them.

Your mushrooms are a bit too old and beat up for me to confidently identify, but as you believe your dog ate some this is an emergency situation. You should post these in the Poisons Help; Emergency Identification For Mushrooms & Plants on Facebook. There you will be immediately put in touch with world experts who will hopefully be able to identify them for you.

It is likely a Cortinarius species, hard to tell which.
