Hi all, I'm new to the forum and my English isn't so good but I'll try to be as accurate as I can 
After few trips I think magic finally found me ???
Habitat: Czech Republic; Mixed forest; altitude ~700 above sea level;
Gills: color: "creamy yellow" (??); dark after few hours
Stem: Length: 4-6 cm Diameter: few mm Color: "creamy yellow" After a while noticeable blue color on the stem
Cap: Diameter: 0.5 - 1.5 cm Color: "creamy yellow" Very clear blue/green spots after a while
Spore print color: Dark brown; maybe black ... but I'd say dark brown (the print wasn't very clear and it was first time I made print). I'm posting a picture, but it's not very good.
Bruising: Blue/green

How to make a print: cut the stem off as close to the gills as you can, put the Gill side down and let it sit for at least 6 to 8 hours minimum 8 to 12 is even better.
I do believe that you are correct, its definitely a psilocybe species. Psilocybe serbica/bohemica.
Are you willing to send me a spore print for microscopy and possible cultivation? I have some I can trade if you are interested.

@stevo: I thought that p. serbica var bohemica is the same as p. bohemica. My guess was serbica var bohemica but I shorted it as I thought it's the same.
I did find new spot besides water stream today as I thought there would be wet ground (I was right). It didn't rain here for like 10 days and it was warm ... and if I'm not wrong they like moisture and cold weather.
Oh and I'm making prints on aluminium foil instead of paper. It should be better as I understand?
Will post pics when I have the time.

Nice find! I'm looking in my country for the same species as well. I have not found them so far. I would very much like to cultivate them and spread them in the local forests by mycelium spawn. If you can send me a spore print by any chance I would be very grateful.
