Is this contamination? PF tek B+ strain

I used pf tek to make 5 jars, drilled 4 holes in top of each jar that are small (1/8" drill bit instead of recommended 1/4). I also pressure cooked these and left them in the pressure cooker for almost 4 days before inoculating them inside a still air box sterilized with 70% IPA . They were cooked for 90 minutes, sterilization tape turned black /indicated sterilization. I cooked them with the jars fully tightened down and foil wrapped around each jar with sterilization tape holding the foil in place. I'm thinking that maybe the foil being taped around prevented the steam from getting into the holes to sterilize the substrate or maybe the holes were too small for steam to get inside the jars. Any comment on what could have gone wrong or if this is even a contaminant? 1 of the 5 has a similar looking growth but no green color.

Based in the green coloring? What do you think that is? I'm assuming my mistake was the small holes in the lid, and leaving the jars in the PC for too long.. Thoughts before I attempt a second round? I have synthetic filter disks and injection ports now, but from what I've read those won't be of any help with pf tek, since it's the dry verm that's acting as the 'filter'. Thoughts?

myc is white. any other colors are not myc. green is mold. so you toss.
bad inoculation technique or dirty inoculate. or both.
id start reading here.

I'm wondering, should I just take my spore syringe, inject the contents into a sterile 10mL vial, and attach the synthetic filter disks/injection ports to my pf tek lids? Then just use a new sterile 1Cc insulin syringe to suck up from the vial, inject into the jars, toss the syringe for a new one and repeat? That way, there's no chance of contaminants getting in...
But what material would be best for attaching the injection ports and 90mm filter disks, and should the jars be tightly closed when autoclaveing?

Those jars are screwed. Your setup is fine it seems your only problem is waiting 4 days to inoculate. I've had many jars where if I leave them out for even 2 days they'll start to smell sour.
You don't need SFD/injection ports right now with BRF jars. And definitely do not mess with your MS syringe like that...
