What's the problem with this mushroom, never really seemed to form a cap/veil, or extend upwards....

It looks edible. That's a mutation of some kind. Doesn't look rotten.

The mushroom is mutated.
The other thing wrong with it is you have your cakes in aluminum foil. This leads to standing water and contaminants. I always recommend just using upside down jar lids. They get as close to the perlite as they can, and make full use of the sgfc and the casing layer (roll) on your cakes.

Mycelium eats foil.
You roll cakes to create a casing layer. This makes a 1-2mm evaporative microclimate up and down the cake. This is the 2nd best pinning trigger next to light. The whole point is to utilize your roll/casing the best you can, and use the SGFC the best you can. The closer it can be to the perlite without obstructions the better.
A proper fruiting chamber and having them on jar lids make them fruit up and down the entire cake.

Do what you want I guess.
It's also easier to move them.. and every time you're misting you're leaving standing water in open air. This is never a good thing.

If you insist on foil at least do it right and make discs for them to sit on, not cups or whatever it is you're trying to accomplish there.
Edit: I can't see in the picture but do you even have holes drilled in the tote? Poor FAE and too much/little water also makes mutants.
