Mold. Surface infection in all jars tells me the filter is not working, and your jar area is dirty.

That, and I'd say the spawn was possibly contaminated from the start. Look at the signs of bacteria lower in the jars, specifically pics 4 and 5. Dirty inoculant, bad sterile technique during inoculation, and/or bad sterilization of jars.

Mold spores die at the pasteurization level, that would mean he did not cook it. Filters, assembly and grow area. And also inoculation method.
60°C for an hour is enough to kill spores.

A filter disc or possibly tyvek is going to provide more consistent results than a polyfill filter, and is also more user-friendly. I would switch to filter discs if possible, and also double check your inoculation procedure. Are you using a still air box?

But im guessing more the inoculant or area. I would def get some filter disc if can find it and if more user friendly.

You need to use a still air box. That should help. Also, PC for 90 minutes minimum.

In that case the filters might be ok, but I would still consider upgrading them. You accidentally infected the grain when you inoculated it open air.
However you said you have a SAB now, so good luck

I'm pretty sure you can use the fabric from the painters suits sold at home diy stores. There's a word for the fabric that's escaping me but I'll try and find it and edit this for you.

Would any of you consider using self healing injection ports along with filter disc?

Tyvek, micropore tape, HEPA sheets. I would Google around or check the vendor list.

I can for sure find micro pore tape locally but ive read not to pc it cause it would melt ? And would you use to cover your holes on your jar then inoculate it and cover with another layer or two ?

If you had a SAB going, issues like adding filters after inoculating generally stop. SAB's add flexibility, and offer after cooking assembly. #1 tool to have.
Even a large 80 liter clear bag and an elastic band can work.
A flow box, combined, is the best you can make.
