Every form that comes into being dies. In theory anything that can die can be born again. The caveat is that the qualities inherent in form are not born and do not die. They exemplify reality and provide the possibility of form.
I suppose consciousness can be destroyed per instance. They'll never get all of us though.

Consciousness is not a 'form'. It is the medium in which forms are perceived. Thus it is not subject to laws such as "Every form that comes into being dies."

I think of it as a process. Processes aren't forms in a strict material sense but they do arise and dissipate.
I think perhaps the propensity of reality to produce consciousness is something that goes beyond form. Reality also tends to create stars. The stars aren't eternal but the ability to manifest them may be.

I like it except for the manifestation of stars, do you mean accommodate star formation? with manifest, I think of agency.

Yes, formation would also work. Manifest as a verb means to reveal or display.
There's no logical reason to suspect consciousness is any different than any other phenomena. It comes and goes like everything else.
Consciousness reincarnates. The potential for it is eternal. It's formations are not.
