Green Mold at Fruit Time, never before?

I'm nearly lost on this one, failures are mounting up: Thai Koh SS from spores into culture, isolated 4 distinct members into culture then expanded them and took the most viable culture for stock. It grows like crazy, rapidly through dishes, grain, then into 50/50 coir-vermiculite but when its time to open up the mono-tub for fruit here comes the green meanies. At first I thought maybe too much water so did .75 gal, still went green at same point. This last run I sterilized the 50/50 and it did the same thing. I use a laminar flow hood for all transfers and do not have contamination issues with any of my gourmets. I have not heard of trich being in culture and then undetected through-out the life cycle. It is probably something stupid i'm missing but when you do so many reps per week you shouldn't screw up the same deal the same way over and over while getting the others right.

Shake your spawn jars at 100% colonized and watch the recovery as well. Bacteria is the #1 cause of trichoderma

Sorry to bother you again, i'm still lost on this one. started with a new spore print and got the same exact result, nice cultures, rapidly growing grain spawn, 50/50 tub grows through then bam, green mold as soon as i bring them out for fresh air and light. they are in the same grow room as my shiitakes and oysters, could this be linked? I finally figured out how to paste my pics

That mycelium looks weird to me.
The spores are whats green, it can be there but just not have sporulated yet. The culture looks bacterial or something, and the weakened mycelium is an open door to tub ending contams.
