We all know how to vent. Pressure cookers just suck at it.
Autoclaves have probes that measure the discrepancy between pressure and temperature and inject more steam(rather than water boiling in the chamber) and also use vacuum pumps to pulse the pressure to try to remove air. Even though water expands over 1000x when it turns to steam air is probably trapped in our cookers even after what we call proper venting.
I had this small tuttnauer autoclave in this lab i worked in and it wouldn't start the cycle till the probe read 121C but sometimes it would take 30-45m to get there with the software keeping the pump trying and steam injection hissing away. And that was with just 8 500ml media bottles in it. It would have no problem sterilizing glass pipettes though.

Am I supposed to add the weight after. The water has been boiling for a minute? I've just left the weight on from the start...

Then you never read the manual. Doing that will guarantee entrapped air.
You don't add it after its been boiling a minute you add the weight after its been boiling at least ten

Ill let the pc vent till the gauge reads anything over 0 then ill add the weight till its over 5 and take ot off for about 30 seconds or so

Now I'm not sure what to think dude, maybe it was not caramelization, but strange that it only happened that one time I pc'd at 20psi. One thing I'm positive is that I always make sure the malt is completely dissolved. Not only that, when all the no-pours solidified before introducing them in the pc I didn't see any weird sediment in them, just perfectly clear MEA.
Well enough of this fagotry! I must be boring ya to tears. Better tell you nice stuff, like I could get your APE swab to germinate, cleaning it up right now

Dude finally! 
I still wouldnt rule caramelization out but i have definitely seen the floating flocculation many times even at 15psi. Ill do some tests tonight or tomorrow to see for myself if the higher psi is responsible. Ill prep two bottles the same way

I think mixing malt with cold water helps it disolve better than warm or hot.
I always mix the dry ingredients in the pan and then pour in the cold water and stir in. I never get any chunks this way. just make sure it is fully mixed not stuck on the pan brfore applying heat or that create chunks as well.

They can sterilize at a high psi easily. I run mine at 20 psi. But the real advantage is things like pulling a vacuum and with the electric ones the ability to not need a stove. Just plug in the wall in whatever room you like.
