probably genetics..
seems a little dry in there, but what you are asking about is probably due to genetics.

Poor conditions. The cap is rotting from pseudomonias. That is caused by poor FAE allowing for standing water on the cap. Poor FAE is also making the fruits lanky and the stipes fuzzy.
Take the kit out of the choke bag and into a proper fruiting chamber like a sgfc. Or at least cut some slit in the bag or something.

Should rotting cap be removed will it be a problem for the others? And wouldn't a slit in the bag negatively effect humidity?

looks like i was completely wrong =p
i need to learn more about things like pseudomonias. No idea what it is or how it happens haha. to me, it just looked like the stem was a little thin right before the cap. i didnt look at the first pic zoomed in, but still thought it was just a genetic thing =p
but definitely listen to Pasty. He knows his stuff!

Now that I look at it again indoors (damn glare on my screen) it might not be pseudomonias. But certainly it's in bad shape. Pretty much dead, it's not going to grow no more.
Mushrooms need a lot of fresh air. More than mammals do. How long would a mammal survive in that bag?
Stop wasting money on kits.

Caused by a grow kit fruiting chamber. 50% humidity would be an improvement if they could at least breathe

I thought it was pseudomonas too until I saw the verm on the cap xD.
It's definitely suffering from bad conditions.
