It's true. Anything short of perfection in your sterile procedure will lead to a nasty mess. Sometimes a small amount of bacteria in a jar can still be used to make a tub, but it's best to avoid that.

Hey everyone, thanks for the responses.
I followed the steps outlined in this guide
I used gloves to cover the holes in the lids because the foil kept tearing and I though gloves are sterile anyway.
I mixed the brown rice flower and verm as per the ratio in that guide, but double as the guide is for 250ml jars and mine are 500ml.
After adding the verm, water and rice flower into the jars I placed them in the pressure cooker for 45 minutes.
Afterwards I injected the spores with a needle in a sterilised box with gloves on, the needle was heated.
I didn't skip anything, and I see I grew some mycelium so I'm still pretty happy with my first attempt.
Any tips for the next try?

the foil kept tearing through... are you leaving the foil on your jars after inoculating? that's a no no. your clean box should not have the gloves attached either, Still Air Box > Glove Box.
500ml is not optimal for PF tek cakes, as they're more prone to stalling.

After looking at the link you followed, it does not appear as though you followed it. You can see that your jars are not full and the verm on top has become saturated. The purpose of the DRY vermiculite on top it help prevent contamination from making it to the substrate. It is also VERY important to wipe away any excess substrate from the top inch or so of the jar before adding the verm, otherwise you leave a road for contams to travel on.
I'd suggest you acquire the proper 1/2 pint jars before trying again.
Personally, I have never had a properly prepared PF jar go bad on me.

Yes, I was lifting the foil, injecting the spores and then when I put the foil back down it was tearing, so I just put a glove over the lid instead.
So the point of the foil is to stop water getting into the jars while inside the pressure cooker? After pressure cooking the jars, I remove the foil, take off the lid, add the dry verm layer, reattach the lid, inject the spores and then cover the holes with surgical tape?
Also I have sourced some wide mouth 250ml jars for the next attempt.

Thanks, now that you mention it I think the mistake I made was not sterilising the dry verm layer, do I do this in the oven?

No. You punch holes in the lid, leave the lid on, sterilize, remove foil, inoculate through the holes in the lid, and leave it the hell alone. You never ever remove the lid after sterilizing unless it's ready for birthing.

Go to http://www.mushroomvideos.com/ and watch the "BRF/PF Tek" videos, all 4 parts. Pay very close attention, follow the process EXACTLY and you'll succeed. I know it can be a pain to go out and buy something that is almost, but not exactly, what you already have. But, these thing have been chosen by long time experts for the reliability they provide.
