Stupid Pressure Cooker

So a friend of a friend just pressure cooked 2 WBS jars, and one of them got all messed up....

He let the pressure cooker go for 80 minutes, with 4 cups of water. He's got a little metal riser plate, so he put the two jars lying down, one sort of on top of the other since it's a small PC

So around 75 minutes, he noticed that the PC was letting out a lot less steam than a few minutes before... So he took the PC off the heat, and when he opened it up, the bottom half of the bottom jar got all cooked and expanded so much that the WBS squirted out a bunch of gunk.

Should I assume that not enough water was added? Since his PC is so small, water rises quickly, so he didn't want his sideways jars to have water seep in (polyfill with coffee filters on top). Or is it because the jar was at the bottom?

About 5 minutes before he took it off the burner, it started smelling like WBS in the kitchen, which is why he's assuming it's the lack of water that cooked it

Cook it less.....i've had no problems with grain at 45-60 minutes...doesn't seem to contaiminate.
