I use mike's stuff from www.out-grow.com
The PDA and MEA worked good for me and I don't have to meet a minimum order of like 5lbs.

I use MEA, can't tell you if its the best but its worked for me every time.

Have you considered getting agar powder and dextrose, potato flakes, yeast extract, malt extract, LME, or peptones to make your own blends?
If you have everything you need and not pre-mixed then you can decide if you want dog food agar, PDYEA, LMEA, etc for each batch.

I don't use any antibiotics so I can't help you with that, sorry!
Do you feel you need to use antibiotics? Gentamicin seems to be popular but i don't think it comes pre-mixed

Thanks 99% of my contamination is bacterial so I thought this might help the problem

and op, go with MEA. Out-grow is the first stuff I used although my myc came out very thin with the PDA, I think because it was going from low nutrient to high nutrient agar. I have both their PDA and MEA, and I also have fungi perfecti's MEA. I prefer the fungi perfecti MEA.
I also have ingredients to make my own.
also, gentamycin sulfate is mixed at a .05% rate per 500ml I think.
Fungi perfecti sells antibiotic agar premixed.
Personally, I prefer fungi perfecti's agar.

id get the small one, or even get the MEA and buy the gentamycin sulfate separate(outgrow sells it, or maybe shroom supply, I forget). That way you could have the MEA and just throw the GS when you needed antibac agar, just a thought.
You really wont need much of it, and its an expensive premix. Let me know if you cant find it

Telephone Brand: How can you lose with agar so asian it defies rationality. 
Spend that money you would have used on antibiotics on hookers. Then you'll be able to get the antibiotics for free from your doctor next time!

I spawned some popcorn casings and had double-overlay cause I didn't put enough hydrogen peroxide in my automated aquarium mister. I only got one mushroom so I cut off the head part where the seeds fall from and put it in a jar of LC and sprayed it all over a tin of PF cakes I made with gravel, cardboard, and bisquick in my microwave. I think it will be good cause B+ is so potent. Triggered yet? Only a square would say "a cube is a cube."
No, this does not look right...

Likewise man, i hope the cacti are doing good! Im trying to prevent mold with all the rain in california recently.
I think out-grow's PDA does have a lot of nutrients in it, im never quite able to dissolve all of it. I thought it was just me but i can take off my tin foil hat now 
I'll have to try FP's stuff too so I can get a feel for both.
To the OP: bacterial contams are pretty common and something you should learn to isolate away from and prevent during the agar prep and transfers. I would only use antibiotics to save a very contaminated LC/spore syringe or if smearing onto your plates doesn't help after a few tries.
I feel your pain though, it hurts a lot to lose expensive premix on bacteria, i lost about 300ml a few weeks ago. I'm still butthurt  
That asian stuff, is honestly a very good source if you can find it at a local store near you. You can also do waaaayyyyyy more plates and slants for the cost of premix.
