Everything has started going wrong

I started growing Oyster mushrooms (P. Florida or P. Sajro Caju) from 2012 (every year from September to April; when its cold in India) and I never encountered any contamination problem until this year when I started this season a bit early from August. Usually I grow with 25 to 30 kg of spawn in a go as I have to purchase spawn from govt. laboratory which is 350 kms away from my facility . But this year I got a Kg of spawn (P. Sajor Caju) from a new source in July itself and I made 4 logs with them only to notice green mold contaminating them while incubation. I didnt take it seriously as it was a small loss. Later I purchased around 30 kgs (for August crop) of spawn from my old source but to my dismay the result were same. Almost 90 percent of bags got contaminated with green mold. However I managed to save some of the partially contaminated bags with cutting off the contam area and spraying with carbandazim solution. I could only earn the amount I spent for spawn with the sale of mushroom and my expenses on substrate and labor etc. were not recovered. I purchased only 20 Kgs. of spawn this time for October crop and hoped for better results this time because the temperature and conditions were much favorable/ almost perfect. But by the time they got incubated, again the same problem has arisen, the GREEN MOLD epidemic.
I wonder what went wrong...
Is it the last year's last crop which was lying in the cropping room for long after the bags had already fruited and exhausted that showed some signs of green mold which I mistook as algae; the reason for the airborne spores of green mold. (But the rooms were disinfected and kept empty for around 2 months before being filled with another crop)
I have noticed green spots developing from the points where the bags had been perforated for GE. I too noticed few mushroom flies invading the incubation room. So is it because of the flies or again the airborne spores. But in few bags the contam appear from places other than the perforated areas.
What can be the reason for the contamination of my crop, though all the methods followed are same as before?

Tough break losing all that, sounds as though either the spawn you purchased wasn't properly sterilized, or you your place is loaded with trich spores.
You could buy the ingredients yourself and use agar to ensure you have good mycelium, sterilise your own spawn, and disinfect your place. Also check the temperature you are incubating at, above 80f and you are more likey to germinate mold spores.

Were the spawn bags closed when they got contaminated and supposedly sterile?
If so, it couldn't be your fault.
This leads me to believe they were probably opened though:

Even if you cleaned the room with bleach, spores could survive or make their way in and can definitely live longer than a few months. Since it sounds like Trichoderma spp. (an actual fungal pathogen) you will want to watch out since it will attack in the stages you aren't generally sterile with such as fruiting.
Unless you spray the room with 2% peracetic acid or the like, you won't kill everything in the first place.

Never incubate a bulk substrate...one mold spore in your incubation area and it's over Do not incubate if you cannot make sure there will not be a single contaminant mold spore
Always pasteurize straw to 65 C or higher for at least 30 minutes, let cool to room temp, then spawn with oyster grains. Leaving a pasteurized cooled substrate without spawning is not good as one mold spore could be getting a head start before you add your mycelium
Mold lives where it is wet humid and dirty. Always clean the room / area where your substrate will be, a clean room is essential for growing without contamination. Good luck

It is actually the straw logs/bags which are getting contaminated while incubation, the spawn I purchased appeared absolutely fine.
