Habitat: Where does it grow? Eg. woods, pasture, state, province, country, altitude, etc. What does it grow on? Eg. soil, dung, wood (dead, living, what kind of wood?), etc.
Gills: Color, attached/not, gills/pores, etc.
Stem: Length, diameter, color, texture, hollow/solid, thin/thick, etc.
Cap: Diameter, color, texture, conical/spherical, convex/concave, etc.
Spore print color: Very important!
Bruising: Color that the mushroom bruises, if any.
Other information: Scent of the mushroom, anything else you think is important, large close-up pictures showing stem, cap and gills.

Last one most certainly looks like a cubensis(that being said, the picture sucks, so don't go by that), but you do realize you just copied and pasted the format without putting in your actual information?
That doesn't help us at all.

I think the one in the last pic that is not A. muscaria is Amanita phalloides (from colour, general appearance and OP's information that it had a volva).

You're in the Southeast US, aren't you? The blue trumpet thing looks like Lactirius paradoxicus. Death caps don't commonly occur in it's range, I think: you have some other Amanita. And an A. muscaria, of course.

I misspelled the name of the blue milk cap. It's Lactarius paradoxus. You'll have more luck finding that.
I'm not sure what a "well epicenter" is? The Death Cap is native to Europe. It has occasionally been introduced into the US with trees transplanted from Europe (so they could appear anywhere). In CA they've naturalized and are somewhat invasive, but not as much in the east. All eastern US observations of A. phalloides in mushroomobserver are north of the Mason-Dixon line.
