Ever happened to you, or anyone you know? If so, what happened? Outcome? Treatment? Thank you, in advance, for your response.

No one I knew, or myself for that matter, has ever suffered from this. Of course, everyone's biology is different so it could happen to someone, but it seems to me to be highly unlikely.

Maybe immune system medications or nutritional diets could help you. Anaphylaxis is basically an allergic reaction to something at it seems like you are allergic to something in a psilocybin cubensis mushroom.
Looking into the symptoms you've had may help you to deal with/overcome them. https://en.wikipedia.org/wiki/Anaphylaxis

Thank you for your response. I am writing an article for a local "zine" on this topic. The reaction was not my own, but did happen to a good close friend of mine- making this inquiry not only educational, but personal to me as well. Symptoms of intolerance appeared after first ingesting them, although assumed them the normal response. Sometime later they were consumed again, in a smaller amount, but the reaction worsened. Eventually, to the last episode. Anaphylaxis. IgE. The reaction physiologically, coupled with the psychedelic effects from the mushrooms, induced a state of panic/shock where loss of consciousness and "near death/actual death" experiences occurred.
Any links to similar reactions or comments on similar experiences welcomed. Your information can help others. Thank you.

How much was the dose he took? You can bypass most nausea with a resin.

Fairly low dose, comparable to what I've read- ~5 grms freshly harvested, partially air dried. Be advised that nausea is not the paramount concern- but a severe, life-threatening reaction. What information/links do you have supporting? Useful for my article. Thank you for your post.

Wierd. This is the first I've heard of an allergy to mushrooms. Were both doses from the same batch/guy. It could also have been an allergy to something that was on the mushrooms. I suppose it's possible to have an allergy to the mushroom or spores themselves though. Does the guy have any other allergies or similar medical history?
It looks like what you're asking for hasn't been studied very much at all. Here's a link on general mushroom allergens, but it seems to focus on allergic reactions to inhaled spores from some common mushroom families, and it's probably not going to be very helpful since it doesn't have much to go on:
http://www.ncbi.nlm.nih.gov/pubmed/3278649
Here's a link that mentions some stuff done in Finland studying mushroom allergies as part of an occupational hazard or something:
http://www.evira.fi/portal/en/food/manufacture+and+sales/vegetables/harmful+reactions+caused+by+mushrooms
And here's a link to a forum where people are bitching about their mushroom allergies:
