Well anyone has the choice to make really high quality, obscure stuff, but like you said, quality in the modern day doesn't necessarily equate to sales or even people noticing you. I think honestly, the most genius artists of our time are the ones able to remain to what they are trying to express, yet still doing it within an accessible "pop culture" type of framework.
In music, for example, it's very much possible to express the same feeling in a pop song as a doom metal song or something. The artists that are able to do that are real geniuses because they remain true to their expression, yet do it in a way that is still accessible to the masses. A good example of this is a band like The Cure....their subject matter is gloomy as all fuck yet they were able to craft masterful pop songs that still catered to the mainstream.
Point is, no matter how good you are, your art doesn't have as big of an impact if people don't know who you are. The real geniuses stay true AND know how to hook the masses.

If you don't put some kind of image out there, modern day people aren't going to give a shit, plain and simple. You can still be true to your art while creating a public persona. The artists that release stuff and then just disappear into anonymity...well of course nobody's going to notice that.
If you don't have some kind of a reputation in the eyes of the public, people are less likely to care.
