it might be.
the veil looks like amanita, the size is about right and it does appear to be growing on a tree root, and deathcap is a native to these parts as well as several other amanitas.

It's definitely an amanita. It has that creepy mountain pattern on the stem.

Volva could've rotted off or been munched by slugs... or possible Limacella sp.? They can look very Amanita like (esp. L.guttata).

i dunno, after looking at some pictures i am more and more of the opinion that it is a deathcap.
i see lots of pics of deathcaps splitting their hoods as they curl up, and i saw the same mushroom with split caps like that on the spot.
slugs and snails are quite likely btw. we got lots of those out here too.

some more have popped up, but unfortunately i didn't have my phone with me.
i did check some more of the deathcap looking ones and on closer inspection i found they do have a volva hiding beneath the moss.

That solves that one then. It's amazing it's responsible for so many poisonings, I can't think what someone would think they're eating.

My guesses 1. Laccaria (maybe L. laccata) 2. Amanita 3. Russula 4. Amanita
