Weird pain after emptying bladder

Okay, well there's pretty much no chance I'm going to talk to a doctor about this unless it is absolutely something serious (not "could be"). I have my reasons, money and embarrassment are some.
But I'd like to know if anyone knows what would cause this, maybe there's something I can do it to prevent it.
So here's the deal. Sometimes when I take a piss and had to go really bad (but not all the time) I'll get this excruciating pain in the gooch. I feels pretty much dead center between the sack and the hole. It does not come from the abdomen area, but at or just below the surface of the gooch.
It is incredibly painful, I honestly wouldn't be able to tell you if I had just got stabbed there, or if it was just something going on in my body, except for the fact no one gets close to me with a blade while pissing. It's just extremely painful. It literally feels like someone stabbed me. It happens at literally no there time. I've had this problem for a couple of years now, and had it dozens of times, it has never once happened in any other circumstance.
Ideas?

Fuck if I know man. Just talk to your doctor, that's what they're there for. They see shit that would make you cringe on daily basis so talking about some pain while you piss is definitely not out there for them

I know it's no big deal for him, it's a big deal for me though. But that's not really the only issue. I just wanna see if anyone could shed light on exactly what it is. It's kind of strange. Googling around doesn't tell you much. Lots of hits come up if you search for it, but when you read, you realize it's not the same thing.

I don't mean this to come off as dickish, if it does, sorry. But I'm just simply not going to go to a doctor unless someone can tell exactly what the issue is and that it's serious. Short of that I'm not going for various reasons. The embarrassment factor, the fact that we're getting pretty close to not being able to afford food and may end up homelss sometime in the not so distant future.. and a few others. "Free clinics" are not an answer, either. I've been through them, was actually the only healthcare we could afford ever. All they do is guess and throw you some random pill and tell you to come back if it doesn't work. Which it never does. You go back a half dozen times before you just give up on it all together. I'm not going to get a diagnosis or real solution there. I didn't need to make a thread to be told to go to the doctor, if I was gonna go, I would've already. It's not as if I don't recognize this to be unusual. Even if it means no one responds, it's fine to just ignore the thread, if the only thing anyone can say is go to the doctor. I'm just making it to see if anyone could give any kind of definitive or reasonable cause.

Na it wasn't dickish, but the fact of the matter is asking what's wrong with you on a drug Forum or googling it when you're experiencing extreme pain is going to do nothing. Nobody will know what's wrong with you until you get a professional and they look at your medical history. Have you had any UTI's?

No UTI.
And yes I get it's a strong probability no one could answer the question, but it never hurts to ask. It's not even a very common thing. Maybe once or twice a month at most.

Could be kidney stones that you are passing, could be urethral cancer. Go to a doctor. If its not fungus related then we probably can't help. There are clinics that serve those who have little means to pay for a doctor, and some cities even have free clinics. And remember, your doctor has definitely seen some way nastier stuff, so don't worry about being judged. Also, be up front about your issue, don't down play it or beat around the bush. For an accurate diagnosis the doc needs as much detail as you can give.

It can be financially scary, but our health is probably the most important thing we can invest in. It's unfortunate it's gripped by the tendrils of capital, like most basic needs...
You won't like this answer, but ANY time you have an excruciating pain that makes it difficult to do everyday activities, you probably need to see a doctor.
In WA, we provide health benefits for low income people (make under $1300 a month through "WA Apple" program), and they can be retroactively applied to a doctor visit. I learned about all this when I finally went to an ER over chest pains and they wanted $5000 for cat scans and stuff. If I hadn't qualified for that, there were a number of grants available from the hospital itself that would cover me for being low income.
Finally, DO NOT BE EMBARRASSED! It stays confidential with your doctors (legally, it has to) and I bet you they've seen worse shit than anything you could throw at them. You aren't trying to get a job with them, or fuck them, so who cares what they think. That's not what the relationship is about.

So I'll be the first to actually comment on your pain. I too think have had the same pain as you. When I spoke to a friend about it he said something to the degree of kidney stones. They can hurt like a motherfucker, pure fire going through your urethre/veins. This may be it, in which case you need to up your vegetable intake as well as your water consumption.

Varies greatly. For long periods it may be well balanced and healthy, and then I'll go on a kick of deep fried foods for a few days. Only consistent non-healthy aspect of my diet is that I vastly prefer meat to vegetables. I won't eat a dish unless it has a good amount of meat to it. But I eat plenty of vegetables with meat. The more common ones are Kale, Broccoli, Carrots, Potatoes, Asparagus and Peppers.
Would a kidney stone stay lodged for years? I don't actually know how long it's been going on but it's been at least a year. Although I'm sure it happened before this point, the first time I actually recall it was the drug test for my job, which was almost a year and a half ago now.

This is great advice. Are you in the US, OP?
Seriously fuck being embarrassed by that, there's nothing to be embarrassed about. It might be something really simple that you could just take a couple pills and fix, it might be something serious.
If I said you'll potentially become sexually impotent will that make you go to the doctor?

No. I'm at a point in my life where sex doesn't really interest me anymore anyways.
And like I said earlier, I didn't need to come here to be told "go to the doctor" I already know I should go and am already refusing to do so.
I was honestly just curious if anyone knows what it would be. It's not a general soreness, it's a pretty specific pain, figured I stood some kind of chance of someone out there knowing what it is.

Naw even doctors sometimes can get stumped by this stuff, but they are actually equipped and able to run tests to rule things out is the main difference, we can't do that. There's too many possibilities.
Honestly I have a hard time understanding why you wouldn't go if it is that bad, sounds really unpleasant, but nobody can make you. Imagine how much of a relief it would be to just take some pills and have it just go away, that could very easily happen. That for me would be enough to get me off my ass.
One time when I was a kid I had pinworms which are little parasites in your lower GI tract that cause extreme itching and irritation. It was very embarrassing to me, and I had no idea what caused it, so I just tried to tough it out. After a while though I just couldn't take it so I went to the doctor and found out it was easily treatable, I took like 2 pills and it was just gone the next day. Sometimes we have to do things we don't want to do, I certainly hate doctors, but nothing beats that sense of relief of having something that really bothers you get resolved.

I have my reasons I won't go, other than ones already mentioned. Another is my alcohol consumption may have to come up and it may be important to not lie about it like I have before. If he finds out how much I drink, I guarantee he'll take me off my sleep meds.
If it were a regular thing I'd probably just go. As it is now, it happens once every couple of months or so maybe. It only lasts about 30 seconds. It's just a minor inconvenience for a very short time. I mean yeah it hurts, but I've dealt with a lot worse pain in my life and didn't go to the doctor over those, either.
Part of it probably has to do with how I was raised, which was I didn't go to a doctor or the hospital unless I was bleeding to death or something. I'm just kind of in the habit of not going to them now. I go when I really have to, when I don't absolutely have to, I don't go.

I'll try to be conscious of it, but I don't really eat much salt (other than what exists naturally in whatever food I'm eating). Actually I'm very salt intolerant. Saltiness is a flavor I despise above all else.

kidney stones will afflict your lower back and abdomen, as well. It's a sharp, but lingering pain, typically.
You can get funding and you have complete confidentiality, but if you are saying it's only once every month or so, for a short time and doesn't impact your life, then who am I to say?
Do your sleeping pills interact with alcohol? If so, there are other sleep aids out there that won't. Your doctor would switch you. If they don't interact, why would he take you off? You don't abuse them do you?
