What is this bubbly, soft area at top of T. Pachanoi?

While inspecting my cactuses this morning, I noticed my T. Pachanoi seemed a bit bubbly and shiny towards the top.  Maybe a tad soft also.  I'm not sure if this might be from over watering or too much fertilizer.  Any thoughts?  Thanks!

I can't really tell what's going on. I think I see some bubbling of the skin, maybe it's starting to rot, or there's been a phytotoxic reaction to being sprayed with fertilizer or something else. I see a white film on it, which makes me think it's being sprayed regularly. It might pay to stop doing this, it's not needed, the plant gets all it needs from it's roots. I'd also not water it anymore and observe what's going to happen.

It's also nice to state where in the world you are or at least what season it is.

Midwest, USA.  It's Summer, temps between 55 degrees F at night and over last couple weeks maybe 80ish during the day.  Its in a place where it gets sun from about 11am-7:30pm.  I have been watering it frequently but the soil is extremely well draining.  Ill hold back from watering it for now.  I sprayed it recently with Pyrethenins and Neem Oil because there was little bugs eating them.

Both neem and pyrethrins can be troublesome if plants are sprayed and left in the sun, and can erode the waxy protection trichs have, exposing tender flesh to UV. Pyrethrins can be quite destructive alone to plants if the concentrations are a bit high. Rosemary oil might be a gentler alternative for preventative sprays. It is best to spray in the evening as the sun is setting, and a rinse afterwards might help too, allowing them to dry before sun up.

Hard to say, ive never seen this. Perhaps too much water and the skin will split soon?

Wow.  Thanks for that information.  Just to mention, I've covered my Bridgesii, Dragon fruits, and Cuzcoid one and none of them were affected.  Im running outside to wash off my T. Pachanoi!
