Did my myc die?

I injected a quart premade grain filter bag with a spore syringe, and let it grow in a 75 degree dark environment.  The myc started to grow, and each grain had about 25% of its surface area covered in myc.  The myc bag never fully colonized, and now the myc is flaking of the grains.  Is it dead?  Any idea why this happened?  I see zero evidence of contamination.
