Has anyone ever seen this happen to a loph seedling before?

I grafted some lophs about 3 weeks to a month ago and almost all took pretty quickly but this 1 took a lil longer to start showing new growth. It started to grow a bit taller but about a week ago I noticed it started to split down the center and this completely new head started growing from the tip.

It does not seem to be growing like all the other pups I've seen grow from my other lophs as it seems to be producing a whole new single head of some sort.
I was just wondering if this is unusual or common? I'll keep everyone updated as it grows though

I have the exact same thing on a caespistosa pup.

Same exact trait on one of my caespitosas, plus I had graft that started new growth ftom apical meristem pup

I have a very similar situation. I grafted these four lophs about a year ago now. The one with the pachanoi stock did not make a good connection and did not put on any new growth until about two weeks ago. I was about to toss it in the trash until I noticed the top part was finally growing.
This was taken a month ago-

the apical meristem was damaged. It is growing a new crown to heal itself. The damaged part will grow underneath and it will look beautiful.

Cacti are just... weird, man. Mine started sprouting babies from the base.
