Found #1-3 in a state park after heavy rains and #4 in mulch at bottom of a big hill at a hospital.

The brownish one or last one looks a bit like a lepiota sp. #3 maybe lactarius sp.

It's actually like rusty colored bumps/scales.

Yeah lots of leprosy species exhibit that scaly texture on there caps. Maybe a TI will chime in and identify.

Edit ^ lepiota, auto correct sucks. Although leprosy kinda works in this scenario.
