Cannabinoid Hyperemesis Syndrome (CHS) (Long, But PLEASE Read!)
    #21953462 - 07/16/15 07:36 PM (8 years, 6 months ago)

A little background information before the main story. I am 19 years old, and I have been smoking weed every single night for the past 3 years and 1 month (I've been keeping count). I use weed to get high, obviously, but I also use it to sleep at night and it helps me get hungry. For the longest time I've basically convinced myself that I need weed for these reasons, mainly sleep though (pretty sure I have bad insomnia). The other important things to note are my health issues. Back in the 3rd grade I think I got the flu or something the day after a track and field meet, and I was throwing up all day. It was a pretty scarring experience for young little me. Ever since then I can't stand puking or anything related to puke. About a week after that, I started feeling this nauseous feeling in the back of my throat ... ALL THE TIME! This hasn't stopped ... I'm 19 now. Sometimes it's bad, and sometimes I can barely feel it (but it's still there, it's pretty much ALWAYS there). The other thing is I have been having regular diarrhea since the 7th grade. I'm not sure what happened, but all of a sudden I started having diarrhea every day, and I never bothered to get it checked out until a few months ago, I'll get to that soon. This background information is very important to keep in mind for the rest of the story.

So, the main story. It was the day of my last exam, April 21. I had just finished writing it and I was driving home. I was VERY hungry, and I usually stay away from fast food, but when I get hungry like that I usually go to fast food, so I headed for McDonalds. I got home and made a McGangBang (Junior Chicken in between a McDouble). Halfway through I start to feel very nauseous, but I'm a dumbass and I powered through the rest of it cause I was hungry. Right after that I started feeling like complete shit. I had plans for the day but I said fuck that to everything and just laid on the couch for about 3 hours feeling like shit. I felt better eventually, but that shitty feeling stayed with me. I woke up the next day, and I still felt like shit. This continued for a few weeks and progressively got worse. It is important to note that I started eating less in general because of this. I had upper abdominal pain and nausea that got slightly worse every day, and who wants to eat when you feel like that? I went to see a doctor at a clinic cause my stomach pain got so bad one night and he prescribed Rabeprazole Sodium (Pariet). This drug makes my stomach produce less acid (he thought this was the problem). My stomach pain was resolved for a bit, but it wasn't completely gone.

My stomach started slowly getting worse again, and then the worst day came. I work overnight at Subway, and this one night my stomach was particularly bad. I got home at around 8am, but before that I had my regular after work cigarette and sesh, thinking the weed would help. Around noon, I threw up. About an hour later, it happens again, so I tell my mom we need to go to the ER. We get to the ER and I puke one more time when the nurse finally saw me. She asks a few basic questions to my mom cause I'm puking. When I'm done, she asks me "Do you smoke weed regularly?". I say that I have one sesh every night. She sticks me with a diagnosis of Cannabinoid Hyperemesis Syndrome (CHS). This syndrome is quite rare ... like, VERY rare. CHS, as the name suggests, basically means that weed causes nausea and vomiting. Here is the wikipedia page if you would like more info/a better description (I highly suggest you read this if you plan on answering, its a short read):

https://en.wikipedia.org/wiki/Cannabinoid_hyperemesis_syndrome

So this nurse sticks me with this diagnosis, and I'm honestly heartbroken. I fucking love weed .. why the fuck is this happening to me? Out of all people, why does it fucking have to be me? She says the only way to cure myself is to stop smoking weed, and she puts me on an IV to give me anti-vomiting drugs. Me and my mom sat in a room for a couple of hours while I rehydrate and get the anti-vomiting drugs in me. I conclude that I don't want to deal with stomach pain anymore, and I definitely don't want to puke again, so I take her advice and stop smoking weed. She said after a week or 2 I would feel completely fine. The first couple days after the ER visit were bad, but I was slowly feeling better. After a week, I started smoking weed again because I felt normal. A week goes by and I still feel normal (besides the slight stomach pain, it never completely went away) and I conclude that the nurse misdiagnosed me and weed wasn't the problem.

During this time, I had an upper abdominal ultrasound and a blood test. The ultrasound results were completely fine, no sign of ulcers or anything dangerous. The blood test results showed that my white blood cell count and hemoglobin count was high. I go to my family doctor and she orders another blood test and she schedules me an appointment with a blood specialist cause she thought I might have polycythemia. Polycythemia is when your bone marrow produces too many white blood cells all the time, and it can cause stomach pain if not treated. I get the results for my second blood test and my white blood cell count went back to normal, but it still said my hemoglobin is higher than the average person. I go to the specialist and he said that my hemoglobin is only slightly higher than the average persons and its nothing to worry about. He also dismisses polycythemia because my hemoglobin count didn't reach the minimum for polycythemia, it was actually quite lower than the minimum. I told him I thought my smoking caused polycythemia, and he also said that I don't smoke enough for me to have polycythemia directly from smoking cigarettes. I smoke 2 cigarettes every day, he said 5 per day is around the minimum for cigarettes to be a direct cause of polycythemia.

So polycythemia isn't the problem, and CHS apparently isn't the problem ... so what the fuck is going on and why do I have upper abdominal pain all the fucking time? I start looking for more answers, and another possibility pops up ... do I have Celiac Disease? Celiac Disease is basically a gluten allergy. So, I figure gluten is the problem! No wonder eating doesn't help my stomach, anything I eat has gluten in it so its probably causing my stomach pain! So I start eating gluten free for a couple of days, and for a few days my stomach felt normal again. There was barely any pain at all. So about a week ago, me and my mom go shopping for gluten free food. I have been eating completely gluten free for the past week or so ... but my stomach still hurts. But here's the weird thing this time. This past week I've been feeling pretty normal in the morning and afternoon, barely any stomach pain. However, right after I smoke weed in the evening, my upper abdominal pain returns. It's not really that bad, but if I put a tiny bit of pressure on my stomach with my hand/finger, it hurts. And also, if I sesh at 9pm and stay up til 2am, around 2am I'll notice that the pain has died down and isn't as bad as when I'm smoking. So I'm starting to think it's CHS again.

Here's the thing though. When a person has CHS, it means that EVERY TIME they smoke weed, they go through vomiting cycles, like you would get with the flu. If they don't go through a vomiting cycle, they get extreme nausea and abdominal pain, and this happens in general anyway because (as we all know) after smoking, the weed stays in their system for a month or so. As long as the weed is in their system, a person with CHS will experience nausea and abdominal pain, the vomiting cycles usually occur when they actually smoke weed. People with CHS say the only way to relieve their symptoms is to stop smoking altogether, or to bathe in really hot water. People with CHS compulsively bathe in hot water to relieve their symptoms.

This is where I get confused as fuck. How can I have CHS when my symptoms BARELY match? Currently, all I have is slight abdominal pain and slight nausea every now and then (but I've had that since the 3rd grade... so its mainly the abdominal pain). First off, when I smoke weed, I don't feel nauseous, and I don't throw up. In fact, the weed actually takes away the nausea if I am feeling nauseous. Secondly, I don't compulsively bathe! Or feel the need to! Sometimes I'll even skip on taking a shower for a day or 2 cause I'm just too busy. And when I do shower, my symptoms don't feel relieved ...

So now I'm just so confused and I'm still as clueless as I was back in April when this all started. I still take Pariet, the pills the first doctor prescribed me. Polycythemia has pretty much been ruled out. My symptoms don't really match with CHS, but my abdominal pain gets worse after I smoke. Apparently Celiac Disease can be "triggered", maybe that McDonalds that one day triggered my Celiac Disease? I also think back to how ever since the 3rd grade I've had chronic nausea, and I've had chronic diarrhea since the 7th grade. How do those 2 things tie in to what I'm experiencing now? Could the nausea thing be a psychological thing? Lately I've been trying to eat healthier in general, and my gluten free diet has been helping with that, and for the past couple days I haven't really had diarrhea except every here and there. It's mostly been solid poops. What's causing that? Is my diarrhea even related to my main stomach problem?

I have an upper endoscopy scheduled for July 30th, 2 weeks from today actually. I don't know what the fuck to do with myself til then though ... if I quit weed (which I REALLY don't wanna do...) I guarantee I'm gonna have major sleeping problems. If I start eating gluten again, that could be very bad for me if I do indeed have Celiac Disease. But I don't wanna deal with my stomach pain anymore ... Should I stop smoking weed for the next couple days to see if it helps?

If anyone has ANY information or advice, PLEASE help me. I've been suffering for 3 months straight and no doctor has been able to help me so far, and I'm starting to have a really tough time enjoying being alive in general. I can't enjoy anything because my stomach pain is constantly on my mind, and I'm constantly wondering what the fuck is wrong with me and if I should be worried.

This is ruining my life ...

You haven't taken a day off in 3 years?
What kind of variety of strains during that time?

Honestly it sounds like the weed actually is causing or at the least exacerbating your stomach problems to some extent. Weird, it usually does the opposite to most people's stomachs.

I feel as tho nicotine also causes some problems.

Also you basically said you felt normal after every time you tried a solution; not smoking weed, eating gluten free etc., which means it's partly all in your head. You expect these things to work, therefore they do work. Which IMO means your stomach problems are all in your head because if they weren't, you wouldn't be able to control them with the placebo effect.

ya man lol I've missed a couple of days here and there when I didn't have a choice, but pretty much ya. I thought that would be the norm around here though lol

I've had a wide variety of strains man. I pick a different strain pretty much every time, and I don't even know how many times I've picked over 3 years lol a lot of times. Why do you ask?

Ever since I've made this post I've really been wondering about how much of this is really all in my head.

I have some updates.

The day I made this post was the last day I had stomach pain with smoking. From that day until now, my stomach has been completely fine after smoking. I believe I made this post about 2 weeks ago, and over these past 2 weeks I've been feeling better every day. I feel pretty much normal now! I only have the slightest abdominal pain when I actually push on my stomach.

So, I've concluded 2 things:

I've either miraculously gotten better and maybe I don't have problems anymore. Maybe it was partly a psychological thing that has gotten better recently. I have been thinking more positively and I've been having better days since I've been feeling better. I've also been eating healthier which has cleared up some issues, so maybe my shitty diet before all this was playing a role.

OR

I have Celiac Disease, and the fact that I've been eating gluten free for the past 3 or so weeks has been relieving my symptoms.

I pray I'm not a Celiac. I feel that there is a high chance that I am though ... I'm truly gonna miss gluten if this is the case lol but it's not the worst thing in the world I guess.

I really think there should be more of a discussion about Cannabinoid Hyperemesis Syndrome though. I GUARANTEE if ANY of you go to your doctor or to the hospital regarding stomach issues related to bad abdominal pain and/or nausea, I am almost 100% certain that they will bring up, and try to blame the problem on, weed. More than half the doctors I've seen have tried to tell me that weed is the problem, but as of right now weed is doing nothing more to me than getting me high (that I know of). I had my upper endoscopy done today, and even before this procedure, me and the doctor were having a chat about my problems and my smoking, and he was trying to tell me all about CHS again lol.

I feel like doctors are gonna really start to press the fact that weed and stomach problems go hand in hand for some people, and it might be something y'all run into some day.

Just sayin

Late to the party,

However I believe I have Cannabinoid Hyperemesis Syndrome. Like you, I had nauseau and vomiting problems in my childhood and teens. When I started smoking weed it made these bad feelings go away. After about two years of daily smoking, I developed signs of pre-emerging CHS: morning sickness, loss off appetite, general yet mild GI problems. About 3 or 4 years into smoking weed daily I had my first acute CHS attack. This hospitalized me every month or two for a full year. The acute attack phase of CHS is the worst thing I have ever gone through. Throughout this year of cyclic vomiting and nausea, I underwent many blood tests, an MRI, 2 CT scans, an ultrasound, endoscopy, and colonoscopy, all of which results said I was amazingly heathy.

CHS comes in 3 phases: pre emetic phase, accute attack, recovery phase...

Notice the signs soon if you think you could potentially be in the pre phase of CHS, and try to cut back on consumption. You do not want to end up in the extreme vomiting and nausea associated with the accute phase (which by the way normal nauseau medications don't work on CHS).

As for me, during my year of cyclic CHS, I was believed to have Chrons and/or ulcerative colitis and/or IBS and/or bowel obstruction, colon polyps, anal fissures, etc etc. but after all tests and procedures I was determined to have none of these problems. The CHS cyclical vomiting episodes have ended, and did not return for a full year. Funny enough, on the one year anniversary, the CHS accute attacks came back. During a two week period I had attacks at least 11 of the day's, all of which lasted over 10 hours. During these attacks, I can barely stand, cannot hold down any food or water, have constant sweats and chills, and the severe nausea and stomach pain keep me from sleeping longer than 10 mins at a time. The most recent trip to the ER is when I was diagnosed with Cannabinoid Hyperemesis Syndrome (CHS), which at the time I had never heard of. I was very skeptical, as I believed pot was helping all sorts of GI and digestive problems. But it appears I may be in the norm who suffer from CHS. For now, I am completely giving up the green until I know it is 100% out of my system, probably a month or two, and ease back in carefully to see if I'll be able to consume in the future.

Best tip: if warm showers/baths make you feel better, and you have some Hyperemesis/vomiting/nausea/ and you smoke a good amount of weed... You have CHS
Buy some hot pepper capsaicin cream

Hi there fellow sufferer lol. So I know I'm hella late but you may have what I have. I have also had doctors diagnose me with CHS when in reality I have CVS (cyclical vomiting syndrome). I know it isn't CHS because I started having my vomiting attacks a full year before I began cannabis use and I started smoking weed to help with all my symptoms. The main difference between the two besides the cannabis use is that in Cyclical Vomiting Syndrome, these attacks have specific triggers like stress, anxiety, lack of sleep, fasting, eating too much, eating right before bed, too much exercise/physical exhaustion, menstrual cycles in women, traveling, exciting events/holidays.. the list goes on and. It's different with each person, for me my triggers are stress, anxiety (it's especially bad around final exams), traveling/holidays. As an episode approaches, I feel my nausea and stomach pain gradually worsen usually presenting as morning sickness for days or weeks, until one morning it's turned into a full-blown vomiting attack. My attacks can last from 2-5 days and I usually go to the ER on day 3 because all my hospital visits have gotten awfully expensive. I do find that maintaining a positive attitude and a sense of control will increase the amount of time between vomiting attacks but I've accepted that there's always going to be another attack and I'll never be totally normal. There is no cure, all anyone with CVS can do is identify then manage/avoid their triggers but even I haven't mastered that yet and it's been quite a few years now.. Like 8-9 I think. . It's possible you've already gotten to the bottom of your stomach problems and I hope you have but just in case you haven't I had to say something because I know how shitty it is to not have a diagnosis. It took me 5 years to get mine and it's nice to finally know. Feel free to message me is you have any questions

Man, marijuana sucks, and I think it's a real drug and should be banned. I'm not a Saint myself and I use pot, but Marijuana is wildly addictive and you can't stop. I prefer red kratom because it is completely harmless, and its effect helps to relax as much as it does from marijuana. Fortunately, I found a service for myself where I can buy kratom bulk. Excellent prices and high quality contributes to the rapid growth of the popularity of this service and I hope this will help many who really need this product.

My first question is, and I searched your post, OP - how many grams per day are you consuming?

I smoke cannabis like, every 3-5 hours depending on what's happening. 1.5-2g a day max, however many bong packs or hits that ends up being. When I go above that I start to have negative effects of cannabis like irritability and just wanting to sleep constantly.

That 1.5-2g a day is optimal for energy, focus, and performance for me.

How many grams a day are you consuming?

Ok first off I fully believe CHS is absolute horse shit.  Second off, you shouldn't be eating McDonalds or Subway.. ever.  I know you work at Subway, but don't eat their food its garbage.  And McDonalds? Even more hot garbage.  If you eat fast food regularly like once a week, that's a serious issue for your stomach health.

Secondly, you need to get on a healthy diet and stay on it for awhile.  Also start taking good quality probiotics - your mom will get you some.  jarro dophilus probiotics is my preferred brand.  For your diet, try starting with a paleo-alkalyzing diet and see how that helps your stomach.

Thirdly, where do you get your weed?  Do you live in a medical state and are going to dispensaries and getting your weed?  Are you just buying it from random people?  Because it's more likely that the weed your smoking is grown with PGR, nasty chemicals, etc. that could be causing the symptoms that slightly resemble CHS.  Now there's a slight chance that you're like one out of... a million people who have CHS.  But I bet not.

Taking a cannabis break or stepping down to CBD is definitely wise.  The other thing I would say is that if you are having any kind of digestive distress (you said some of this stated before cannabis) then you should stop eating dairy, wheat, sugar and processed foods.  This basically boils down to having to eat meat and vegetables.  Give it a couple of months and then slowly reintroduce those foods and see if the digestive issues return.

The dairy thing isn't black and white.  If you are lactose intolerant (as opposed to allergic) then you can still eat butter, cheese and maybe yogurt.  With elimination diets it's always best to eliminate everything you practically can.

The other thing I would add is that intermittent fasting is helpful.  If gives your digestive system time to heal.  Fixes the problem of finding healthy food to buy (you just don't eat dinner and lunch).  In the evening you cook dinner out of meat and vegetables, you are hungry by that point so you eat it all and enjoy it.  Fasting takes while to work up to.  The books by Dr Jason Fung are very helpful.

I work in an emergency department and CHS is very real. Not all vomiting is CHS but its not a made up thing by doctors. Many of them smoke too. Its thought to be caused by the new higher TCH strains because in the late 90's early 2000's it was almost unheard of but as the THC percentage went up so did the occurrence of CHS. We give fluids and Zofran for it but on extreme cases we have given Geodon to control the vomiting.

Hmm, I wonder if I actually have CHS, I tend to get very nauseas whenever I consume more than 30mg of THC worth of edibles or capsules. I've thrown up on three separate occasions (all three times I took more than 30mg) Hell, even a few weeks ago I took a small drop of Rick Simpson oil and about three hours later I was throwing up in a bucket.

If I take smaller amounts, just enough to give me a pleasant high, I usually get no nausea whatsoever.  Also, CBD doesn't give me nausea, I've taken over 100mg of CBD with no problems whatsoever.

I'll do more research on the topic, I haven't even heard of it before until this post.

naa thats not it.. rick simpson oil esp if is with iso or heptane or street grade would make anyone throw up.. also not uncommon for anyone to get upset stomach with edibles esp when hit the u can barely walk and legs feel like 50lbs dosage.  if lasted for full days and only way to get sleep is hot showers then u can worry. or if have lost weight and dehydration.

the flu and track meet issue is something you will get past.

nausea and psychedelics go together for several reasons:
basically if things feel off, off balance, taste is off, temperature is off, emotional approach is off, whatever, when things are 'going awry' NAUSEA is the overall take on the situation.

on psychedelics, especially as they start up, but also during peak, nausea is normal, and things are off, even if they are cuter, shinier, tastier, smellier, funkier, etc. it is off to some degree, and this can be nauseating.

the in-between state : "am I going to barf now? I just don't know" - this is part of playing with psychedelics and weed is psychedelic to a large extent.

scale back a bit, but start to navigate nausea, and please make sure you can do some exercise. If you build strength, especially core (pushups and sit ups - cycling etc.) then you will have more confidence, and your body vitality will offset the nausea somewhat as well.

Ok, so. CHS has very little research behind it. The main thing on my mind when I think about this is outside contamination such as pesticides. Is it something the grower your supply comes from putting in/on the plants tainting it with extreme gardening chemicals not meant to be smoked that you're getting sick from? Have you noticed differences when buying from different people?

I work in an emergency department and CHS is very real. Not all vomiting is CHS but its not a made up thing by doctors. Many of them smoke too. Its thought to be caused by the new higher TCH strains because in the late 90's early 2000's it was almost unheard of but as the THC percentage went up so did the occurrence of CHS. We give fluids and Zofran for it but on extreme cases we have given Geodon to control the vomiting.

I live in an illegal country so have only just discovered CBD.  Where has this stuff been all my life?

The chemical pathways of THC and CBD are related so you don't generally get high THC and CBD in the same plant.  Strains with a balanced ratio are generally not as high a total percentage (CBD + THC) as a strain thats either CBD or THC dominant.  Also outdoor grows are exposed to UV which give it about .5% CBD.

All this adds up to the fact that prohibition drove grows indoors and sent THC levels sky high.  This lead to unbalanced cannabis and the problems we see now.

What have we done to the poor old cannabis plant.  It started off as something quite balanced and mellow.  Prohibition turned it into the equivalent of rotgut whisky or bathtub moonshine.

We should all apologise to the poor old cannabis plant and offer our forgiveness.

Correct.  These drugs are as confusing to your stomach as they are to your head.
