Shake well at around 20%. Then let I would let them finish colonizing. After they are 100% colonized some people will shake once again to see how the grains recover. They usually recover in a couple days. If they take more than a few days then there is a possibility they are contaminated with bacteria. (A lot of times a bacterial contamination will still fruit if it doesn't stall or get another type of contam in it's weakened state)
This info is from personal experience. Others may have different opinions...

yes, once the grains in the jar is completely colonized (100& white) you mix them in a tray/monotub with substrate (mushroom food). then you let the grains and substrate colonize to 100% in the tray or monotub.
once its all white you can choose to add a casing layer (not meant to be food). coir and verm works well both as substrate and casing material.
The verm in grain jars is an old way of ensuring your grains arent to wet. its better to prep the grains right and leave the verm out

Wow. If you've been in the archives stay away from there. You don't want any sort of incubation. Germination colonization and fruiting is best in room temperature. Other than that just listen to the rest of the members

Also, to answer your other question, after your grains are fully colonized(verm or no verm) you will want to mix it with a bulk substrate or case the grains. Either way you can use a 50/50 mix of coir/verm (there are plenty of other options including horse poo, peat moss straw mixes). It is absolutely vital that you pasturize (but not sterilize) the bulk substrate or casing material before you mix with the spawn. Mix the spawn (your colonized grains) and bulk substrate (pasturized verm and coir, or whatever you choose) and leave them to colonize. If using bulk you can go right into fruiting without a casing layer, or you can case the grains and then put them into fruiting. In dry climates casing seems to help. Frank has a really easy to follow grain tek that I recommend for beginners. My best advise is to make a glovebox and buy a pressure cooker if you want good results. Hope you find a method that suits you. Also look into monotubs. They are the future.
