Help WTF is going on with my cakes

It have happened to me a cupped of times now, that I birth a pf-cake that is fully colonized and have nice and fresh mushrooms scent, but just won´t make a single fruit ( like in 3 4 or even 5 weeks) 
When this happens, It is always accompanied by some wery small flies.
I relay don´t know if the cakes lose there life to the flies or if the flies is only there because the cakes have lost there life.
It is my suspicion that I might mist way too mush but hey, I do that only because I think I do it to little, so I guess I don´t know 
I hope to get some help to find out what is wrong with the cakes, and how to avoid it in the future, so I have taken some pictures of a cake that I tear up, to se what was going on inside.
 The cake is sticky and rubbery and have a "dry" and pungent ammonia-like odor.
Also there was some kind of worms in there (look right in the mite of the pic)

what is going on???

One of the most important pinning triggers is water evaporation. Those cakes don't look like they were consolidated when you birthed them, have you been misting?

It's kind of hard to tell what I am looking at. Are those brown rice flour cakes? Did you dunk? Pressure cook. You might need to get a humidity gauge and make sure that your shotgun is keeping the humididt up. When did you last wet the perlite? Also, are you fanning after misting? 3x per day shouldn't be too much unless you live in a highly humid area. Also, if you used vermiculate to case, make sure it isn't the miracle grow brand which I beleive contains fungicides.

Bad odors is contamination. Bugs would be attracted to that.

You shouldn't really have a set amount of times you mist. You mist when it isn't glistening/shimmering anymore, and mist until it is.

Completely unnecessary. Don't rely on a gauge. Just look to see if they need to be misted again. Relative humidity is something to not worry about. You're more concerned with high surface humidity (done through misting), and a lower relative humidity to have constant evaporation.
Contamination always comes from 1 of or a combination of 4 things. Too wet (improperly made sub), bad inoculant (need to use agar), bad sterile procedure/conditions, or bad fruiting conditions (too much/not enough misting and/or not enough fae).
Agar and 2 hour pressure cook cycles saved my ass
