Anyone with majoe back problems? Considering a nerve block.

I currently see a pain specialist for my back and foot, and I have recently had my med dosage increased due to my foot getting worse. This has helped me do more training on my legs(I have really bad wheels), but now the pain has increased in my back.

My doctor specializes in back injections and I went through having the steroid injections done on my facet joints earlier this year, but without success. The next step with the injections would be a nerve block, but I don't think it would be a smart move for me to eliminate the pain completely. I have learned first hand from using pain meds after surgeries that having some pain is a good this for protective reasons.

I fear that blocking the pain too much will allow me to do things that I wouldn't/shouldn't do with the pain as my guide, and I know that once I start these injections I will forever be committed to them just like with the meds. I had a hard time accepting the fact that I needed to see a pain specialist, but there's no other choice once it gets into your back and you've exhausted all other methods for relief. Back pain is the worst because it's impossible to escape in any position you put yourself in.

Any input would be greatly appreciated.

muscular pains are emotionally fixable. Got much help from a dude who snapped my neck and all og a sudden, I could breathe ect..

I don't know how much of an effect that would have on my degenerative disc disease, but thank you for responding.

Facet joint injections tend to be more diagnostic than a long term solution.  For the rare patient, they will remedy the issue entirely.  In my experience, nerve block injections are a step up to a rhizotomy, which will decrease in effectiveness over time--a few years generally.  I have never seen a patient undergo injections forever or a doctor recommend that.  I don't know why that is.  The fact that no doctor I've ever seen has recommended that suggests that that is not really a viable option.  After rhizotomies no longer work, people tend to take a look at a disc fusion.  If you get the fusion, the adjacent discs to the fusion will deteriorate at an accelerated rate causing further complications.  Fusions are a crap shoot.  Sometimes the pain goes away entirely, sometimes it's the same, sometimes worse.  You have to really be careful as the pain management doctors make a fuck ton from injections and they tend to lean towards overtreatment.  I would certainly have an orthopedist supervising your treatment regiment with the pain guy.  Really, the idea is to live a functional life and if injections help you do that, I would do it.  Discs do not regenerate and it's highly unlikely that the pain will ever go away on it's own.

I am quite certain my pain doctor makes his living off these injections, and I got one hell of a sales pitch for them as soon as I walked through his door. I never heard of Rhizotomy before, but from what you are describing, I think I'll play it safe with my meds and not hop on that train.

The last thing I want is to start the ball rolling on medical procedures, but the way it was described to me was as if it was a long term solution. I can't thank you enough for filling me in, and I have got to the point where I have lost my trust in every specialist(I see plenty)I have seen.

I think it was a a clear indicator of how his system operates from what I witnessed on my first visit. They were handing out petitions to fill out because there was some legislation in the works that would effect his ability to perform these procedures. I signed the petition and started talking to others in the waiting room about the injections and listened to one man ramble on maniacally about how they better not take away his injections. A perfect picture of the operation right in my face.

Again, thank you for the insight. :thumbup: It's really hard at times when a good portion of your social network is just a bunch of specialists who are generally company men all on the same team.

If you received even temporary pain relief from facet injections, a rhizotomy would be a good option down the road.  The idea being that if you received some relief, the facet joints are the source of the pain generator.  Basically, they use radiofrequency waves to heat up a nerve to stop it from transmitting pain and it is minimally invasive.  The only problem is it's only recommended for a few years and the nerve will grow back.  There's different perspectives on how long that takes but it's probably months so you probably want to hold off on that as long as possible.  Yeah, finding a good specialist can be a challenge.  The information gap between the patient and the doctor is limitless.
