Yuck. Almost looks like they got eaten by bugs lol. I don't have any experience with contaminated fruits.. I feel for ya man..

I highly doubt it has to do with the casing mix, assuming it was pasteurized properly. The downside of a casing is you create a microclimate for baddies that fell on the surface of the mycelium to germinate. However I've found casings to be great as long as everything was clean during colonization (including spawn). Casings themselves shouldn't cause a problem because it's a place where baddies can't germinate. Mycelium should be strong enough to stop the baddies on the surface so ideally there's no place for contams, if that makes sense.
I'm not sure if it's just diseased or bacterial or whatever but I always blame my colonization process (inoculation, spawn, and substrate) and want to improve my sterile techniques every time something like this happens.

looks like Pseudomonas, bacterial blotch to me. I am trashing it. Just opened The Mushroom Cultivator, Stamets Chilton. I may have answered my own question, but I have never seen it personally. I was getting it too wet I think, just designed a closet and trying to work out the kinks. everything uncased has turned out good though. I messed up somewhere for sure. Everything you said makes sense.

Poor babies 
seems i'm having set back after set back. I fix one problem or get to one stage and then encounter new issues.. going from looking like there's not enough moisture in the inoculated rye berries to condensation and new moisture. I'm guessing any green growing on the berries or mycelium growth is probably conatination.
