I would try adding some sawdust to your substrate in the future. Also try not introducing FAE until after you have a good pin set. My first two attempts failed because I introduced FAE too early.

Hi,
Use oak sawdust with 10% grains in it- fruits really nice but must be oak, others don't work so good.
As for this example, case it with regular soil, unpasteurised, reishi eats shit out of contams. Lower a bit light and fae, introduce when you want conks to form.

Not sure, maybe a pin or two but it isn't yellow spots.
What's with middle right, looks like black mold?

It's probably a dark brown spot not black mold. Looks more like that in OP. You're going to freak the guy out. Reishi often brings yellows and browns onto the substrate after pinning. Supposedly scratch with a fork the areas that haven't started pinning yet and mist a little more will help initiate pinning there too.

Huh? Only oak and only 10% grain? Ganoderma resinaceum or often incorrectly called "lucidum" by venders (citing MrCloudy on taxonomy) will fruit on just about any hardwood, coir, cardboard or even straight grain. Just search for several threads of MrCloudy's experiments with straight WBS. I've done a 50% grain ratio with very good results on hickory. I've also had good results with peanut shells and 50% grain. The key is FAE, when to introduce it and increasing it gradually like previously mentioned. Also lots of humidity is good too.

I use hardwood heating pellets. God only knows what woods they contain and they work great for reishi. I also add quit a bit of oat's and coir to the mix. from what I've seen reishi aren't too picky.

It looks like you are have a few fruits starting to form in there (the bottom right corner looks promising). I would lower the FAE until you get a solid pin set though. Maybe mist a bit. If it gets too dry you will never get fruits.

@ OP
The surface of sub should not be dry and I second the lowering fae.. I suggested casing because it is easier to keep moist. Don't worry about contams, when I get them on substrate while colonising, I just leave the bag and after some time the reishi covers them.
@jumping fish It looked like bunch of spores, but yea could be nothing reishi really makes ugly subs. I fruit mine out of bag, through the holes cut for fae, the shroom grows in the bag and out of the hole - FAE gradient.
