How about Michael Criton's Timeline. I enjoyed the book however the movie supposedly bombed.
Day of the triffids is perhaps my favorite book of all time, but the movie is worse than attack of the killer tomatoes.
Pretty much any movie based on a Dean Koontz novel.

My prize goes to 'Catch 22'. One of the very few good books out of the US that I've read. The film is mindbendingly trite and incomprehensible.

Stephen King - Firestarter It's one of my favorite books, but the movie sucked bad...terrible actors, so many important parts left out...:(
