jars not colonizing completely? pf tek question

my friend used pf tek twice successfully, but then the third time, his jars just didn't colonize completely. a couple showed some growth (up to maybe 40%), others showed none, but none of them were contaminated. he waited a full month before tossing them.
now i'm using the pf tek method (inoculated two weeks ago), and my jars are colonizing, but very slowly. can anyone give me possible reasons his jars may not have colonized completely, so i know what mistakes to avoid?

Your substrate may be packed too tight. Genetics play a big factor in speed. Temperature plays a big factor. Bacteria can cause jars to stall as well.
Your friend probably had bacteria.

^THIS
Also, your jars might be too dry, hence slow growth, or maybe cold temps (below 65F)
