PC failure?

I have a contaminated jar that has been left to sit for ~1 week after having been pressure cooked. I was intending to grain to grain but was unable to do immediately after cooling. Any idea as to the cause?
I have an all american 941 pressure cooker. I am familiar with how to use pressure cookers; right setting on toggle weight, amount of rockage, etc. I recently prepped some grain in the same manner in which I have always done successfully. PCed for 90 minutes at 15 PSI, allowed to cool naturally. I use lids with a 1/4 inch hole covered on one side with a SFD sealed with high temp RTV silicone. A week later I notice the whole jar contaminated with something green/grey.
Thoughts?? I am unnerved by this as I have been having contaminates during fruiting that seemingly come from nowhere. I would upload a photo but cannot think of one that would assist diagnosis; available upon request thought. Thanks shroomery!

the green/grey mix sounds like a bad spore syringe, dont blame the AA...those things are solid, op.
You know to bring the pressure cooker up to 15psi and vent the air pockets prior to capping it to steady at 15psi, right?
Otherwise pockets of air will circulate around the jars and wont come under steam pressure (forces heat all the way to the center of the jar)
But yea, if its a green color than thats after the jars have been sterilized and if its mixed, than for sure its the spore solution that you used.

I would suspect it's something else unless a large percentage of your jars are contaminating when using different innoculant. Plus, All American sets the standard of a great pc, probably something else

Hybrid, I did not even get the change to do a grain to grain, inoculation, or anything of the sort. These are barren grains that contaminated right out of the PC.
That is an affirmative about venting air pockets; always done.
azur, yes, I do a 20(ish) hour soak followed by a 5(ish) minute boil. Then they are transferred to a window screen to dry until they pass RR's toilet paper test.
It was only 1 of the 20 jars that contaminated, which is why I found it so odd.

Sanguin, the contaminate seemed to start all over. I caught it early on and it was evenly distributed throughout the jar. I agree it might be the lid. I am inspecting it now and I cannot see why it would have failed but I am unsure where else to point the finger so to speak. The silicone seal looks solid all around.
Does anyone think there might be "hidden" contaminates in the jars which would yield problems during fruiting? As I mentioned above I've an abnormal amount of contaminates in my monotubs. From seemingly correct smelling, all white, fully colonized spawn jars I am at 6 contaminates out of 20 tubs.

Im confused...
6 jars contaminated out of 20
or 6 tubs contaminated out of 20jars?
Post some pictures of your jars/lids/contaminate, that will help.
Also, if the jars are pulled out of a pressure cooker while still hot they cause a vacume of cold air to be pulled towards them.
If you never got a chance to inoculate them and they're moulding, that is a sign of extremely high levels of mould being present in the air Thadeous.... I always keep a small "control" jar sitting somewhere, once it moulds I clean my place from top to bottom with bleach and disinfectants. Usually they last 3-4 months before mould happens.....Also, the best fight against contamination is a de-humidifier, if they still mould up while sitting out, I suggest moving.
