Grain to wet? Or just not proper sterile technique?

I did G2G transfer with 10 jars and this is the only one that ended up contaminated. (Green mold I believe?) I forgot to shake the jars after the transfer and I think this one was the last jar with the really wet grain from the bottom. I soak for 24 hours and then brought to a light boil for 10 minutes. I think I gave them too much heat. There were a lot of split grains especially in this jar.
Is this from the grain being too wet or from not using proper sterile technique? Any help would be greatly appreciated. Love this hobby, love this site, I love getting spun on caps, & I love some info please.

It's a sterile problem to wet would have caused the grain to rot not sought mold

Yep, you also have contams from the bottom up. that means it was a sterilization issue.

Thanks guys. I have had jars stall but this is the first one that went bad like this. If I didn't learn from my mistakes I would never learn anything at all.

How or can I tell if the grain has started to rot?
