Habitat: Where does it grow? Eg. woods, pasture, state, province, country, altitude, etc. What does it grow on? Eg. soil, dung, wood (dead, living, what kind of wood?), etc.
First let me say I am in South eastern Pennsylvania about half an hour south of philly in a suburban area, I saw these growing mid October and picked them the same day. I did the write up around then, it got deleted and I had not been able to find time to write another until now. I saw a large patch of these growing in the grass from deadwood underground. The rest of the patches that were less mature were growing around the base of the dead tree. The dead tree is a very very large stump that is secluded. It is mostly covered in ivy and is in a healthy grassy area by the road near the entrance to a neighborhood. The biggest cluster was on some buried dead roots about 4-5 feet from the tree. Each group of shrooms, i think 3 groups, just seemed like its own own big cluster. I would grab the cluster at the base and about 10-20 shrooms would come off at a time all grouped together. They mycelium was very healthy.
Gills: Color, attached/not, gills/pores, etc.
The gills are a tan to light brownish color maybe a little yellowish white. They are attached and i guess a little crowded.
Stem: Length, diameter, color, texture, hollow/solid, thin/thick, etc.
Stems were ranging from about 3-4 inches tall all the way to 7-8 inches. Diameter looked to be about an inch. They have white to tan to yellowish stems that look and feel fibrous. The stems were mostly solid i think when i cut the caps off of a few. Sometimes semi-hollow but not really. They were also kind of bulbous down towards the base. Stems were thick.
Cap: Diameter, color, texture, conical/spherical, convex/concave, etc.
Diameter is 1-2 inches up to 3-4.5, the mushrooms were bright yellowy orange that i suspected could be an active gym. They were smooth and not slimy. More flat once matured but still Convex Caps.
Spore print color: Very important! I did not get a spore print but that is because I fucked up. Am I able to rehydrate a few caps in some water and print them later?
Bruising: Color that the mushroom bruises, if any.
They showed light signs of greenish bruising in some of the bigger ones but not until squeezed firmly would they bruise a little bluish. Some had maroon features of bruising without being squeezed or anything. Immature ones had a yellowy green way of bruising. I started drying them when I got home that night and and didn't intentionally bruise any until they were pretty dry a few days later and by then they would bruise a grey/blue or bluish with red hints. This is all just the stem. I didn't try to bruise any caps, just squeezed stems. The caps are dry and a tan color now and I don't suspect they would bruise very easily. I read something yesterday about putting gyms in the freezer to see them bruise easily but i have not tried.
Other information: Scent of the mushroom, anything else you think is important, large close-up pictures showing stem, cap and gills.
Scent is a little pleasant to almost nondescript. The dried caps of younger specimens are a little darker and as they matured the caps would be more yellow to cinnamon in color. The veil is yellow where present and sometimes a little lighter like the color of the gills.
Large one, the color change from the base to the middle of the stem is pretty cool.
 Small one, yellowy green
Group of varying ones, notice the center one has reddish staining/bruising up the stem.
Image i found browsing google linking me to Rogers Mushrooms heres the link if anyones interested: http://www.rogersmushrooms.com/gallery/DisplayBlock~bid~5977.asp
