A vice would make things much easier, honestly I don't know how well it'd work to just hold it. Might be able to figure something out though

This would be a pretty easy procedure, but you would still want to cure the piece or it will be very fragile and prone to breaking.

Do you want holes in the side or bottom? How large of a hole? Unless you have a kiln to relieve the stress you create in the glass I don't see this working. What are you gonna do heat up a spot on the side of the bottle and blow a hot bubble in your face? I say this because there is no blow tube attached to the bottle. Either that or you will have to heat a spot and pick the glass out but without a glass rod or blow tube it'll be a pain in the ass.
If you wanna cut the top or bottom of bottles off this works pretty well.
If you are going to drill the glass you will have a sharp edge that when pressure is applied to it, it will fracture.

you can smooth cut glass with sand paper.

i'm saying after you drill you can smooth the cut by sanding it. like with a dremel and sanding bit after you drill a hole
