I have no idea how to go about this- I did not see any dedicated Artist Conk Cultivation threads so I am going to put all of my research in here!
Basic info and pictures-
http://www.fcps.edu/islandcreekes/ecology/artists_conk.htm
http://www.mushroomexpert.com/ganoderma_applanatum.html
An artist's conk that had a tree fall down that it was growing on.

-
Distinguishing features for Ganoderma applanatum include its unvarnished, furrowed and lumpy, brown-crusted cap surface; its white pore surface, which bruises brown; and its brownish or cinnamon flesh. It is perennial, and the specimens can develop for dozens of years.

Artist's conk grows and survives winters, so I don't think it is all that picky about grow temperature. Searching for an optimal temp now!
http://www.fungi.com/shop/grow-mushrooms-on-logs-and-stumps.html
"By using the dowels to inoculate cut hardwood logs or stumps, mushroom mycelium can be encouraged to grow throughout or colonize the wood. Once the wood is fully colonized (typically 9-12 months) mushrooms will spring forth from cracks or channels in the wood."
http://out-grow.com/fungi-cultures-c-2/10ml-liquid-culture-syringes-artists-conk-ganoderma-applanatum-p-239.html - "Normally this will be sterilized rye berries."
http://www.mushroomexpert.com/ganoderma_applanatum.html - "Spore Print: Brown or reddish brown."

They take years to grow to full size. You pick them in the woods, not grow them. It would be awfully hard to keep a substrate from contaminating indoors over a few years. RR
