Contamination after birthing

Hi all!
I have been using PF tek (RR's instructional video) and have 1 grow under my belt using the SGFC.
I decided to build a PMP for my second grow because it seemed like others have increased yields with this set up.
I birthed about 8 cakes recently, dunked for 24 hours, rolled them in a fine layer of vermiculite and placed them in my PMP.
My PMP has 2 21'' air stones and the tub has been filled with lava rocks. I used tap water instead of distilled, so I'm wondering if this is what caused the contamination issue.
After about 2 and a half weeks of fluffy white mycelium, hyphal knotting and what looked to be like cakes just ready to explode with pins, I picked up one of my cakes to see it positively COVERED with trichoderma on the bottom!
From my best estimate all the cakes were 100% colonized upon birth (I had waited an additional week after 100% prior to birthing) and I am trying to figure out where this mold came from.
Is it possible that the lack of FAE was a contributing factor? The PMP is supposed to be set up to allow for FAE, but I noticed that if I left the top on (a few holes drilled into the top), the CO2 levels would just build and build.
Has anyone else had this issue?? Any suggestions? For the time being, I'm going to be cleaning my grow room (also home office lol) from top to bottom and lysoling the ever loving crap out of it. I'm so sad, but I will most likely have to discard my cakes and start all over again (I have some incubating currently!). I just want to know what triggered this outbreak at such an odd point.

They all had mold and only on the bottoms? That doesn't sound very likely. Are you sure it wasn't bruising, do you have a pic?

Just one cake had the mold. It was definitely not bruising, it seems to have appeared within a couple of days. It was on the bottom of just one cake and some of it broke off onto the lava rocks below. I immediately chucked the offending cake and took the pmp offline and outside for a good cleaning. I rinsed the remaining cakes off and set up a smaller sgfc as a quarantine measure.

If they were fully colonized when you birthed them that one was probably already contaminated. Molds, especially Trich, don't always sporulate before birthing the substrate so the contaminated cake may have still looked healthy when you birthed it.
IMO the SGFC is the far better option but I doubt it would have prevented the contamination in this case.

Thanks for the input. It was just so weird, they were completely colonized and smelled very healthy upon birthing. I'm going to be paranoid from now on lol
